Neurology:神经生理学联合认知量表预测帕金森病痴呆风险

该研究前瞻性入选63例无痴呆的PD患者,在其发展为PDD的7年随访时间中分析其基线的认知功能和脑磁图记录。通过患者认知功能和神经生理标志物以及联合危险因素分析,采用Cox风险比例模型评估PD患者转化为PDD的风险。

研究结束时,19例(30.2%)患者发展为痴呆。基线认知功能和神经生理学标志物均可单独预测发展为帕金森病痴呆。在认知量表测试中,后部执行功能测试和前部执行功能测试是发生痴呆的最强预测因素。在神经生理标志物中,β强度<中位数是PDD的最强预测因素,其次为峰值频率以及θ强度。基线认知功能和神经生理学测量指标两者联合有更强的预测价值,前部执行功能受损联合低β强度与最高的痴呆风险相关。

研究者总结道,与单独应用生物标志物相比,联合神经生理标志物和认知功能评估能显著改善PD痴呆风险预测分析,前部执行功能受损联合低β强度为最强痴呆风险预测因素。为临床治疗以及未来治疗策略的开发提供了可能的获益。
