J Infect Chemother：CRE诱发的神经外科细菌性脑膜炎/脑炎的流行病学和危险因素

本项回顾性研究旨在确定耐碳青霉烯肠杆菌科（CRE）相关脑膜炎/脑炎的流行病学特征，并阐明CRE神经外科感染的危险因素，研究结果已在线发表于J Infect Chemother。

研究人员对每位患者的病历进行回顾，提取20个关于危险因素的临床变量，通过多变量logistic分析对CRE-脑膜炎/脑炎进行评估。

 

结果显示，2012-2017年，神经外科脑膜炎/脑炎的阳性率为7.9%（2947/29605），肠杆菌科占所有细菌感染的6.3%（185/2947）。本研究中共有133个肠杆菌科包括26个CRE分离株。其中，单变量分析显示，CRE脑膜炎的风险因素为呼吸机、菌血症、重症监护室(ICU)入院、医院获得性肺炎和感染导致的死亡。多变量logistic分析显示，医院获得性肺炎和感染导致的死亡是CRE脑膜炎的独立风险因素。

综上所述，该研究结果表明，CRE是世界卫生组织(WHO)2016年公布的最严重的耐药菌之一，CRE引起的脑膜炎/脑炎是神经外科手术失败的重要标志，需要医生立即重视。

患者，男，87岁，排尿困难伴肾功能异常11年，腿部外伤1月余，以“慢性肾病5期，右股骨颈骨折”入院。完善相关检查：X线示右股骨颈骨折；生化及血常规示血肌酐900μmol/L，血红蛋白61g/L，C反应蛋白67mg/L；胸部CT示右肺、左肺上叶慢性感染病变，结核不能排除；ECG示正常范围；心脏超声示主动脉瓣、二尖瓣关闭不全伴中度反流，心脏射血分数（EF）54%。

男，53岁，因双下肢活动障碍4年、加重10 d入院。4年前，头颅MRI示双额颞硬膜下血肿（图1A、1B），MRV示矢状窦闭塞（图1C），保守治疗好转出院，之后反复出现癫痫发作，双下肢活动障碍。10 d 前，双下肢活动障碍加剧，不能站立。体格检查：双上肢肌力、肌张力正常，双下肢肌力3级，肌张力呈齿轮样强直，双侧Babinshi征（+），Kerning征（-）。

Neurology：貌似急性缺血性卒中的半侧脑膜炎

78岁男性，突发右侧无力和缄默，患者没有发热，癫痫发作或头痛的病史。
