NEJM:经导管二尖瓣反流修复术治疗心衰

合并二尖瓣反流(由于左室功能障碍)的心衰患者的预后不良。2018年12月来自美国的G.W. Stone等在 NEJM 上公布了 COAPT 试验结果,目的在于探讨经导管二尖瓣修复术是否能够改善患者的预后。


合并二尖瓣反流(由于左室功能障碍)的心衰患者的预后不良。2018年12月来自美国的G.W. Stone等在 NEJM 上公布了 COAPT 试验结果,目的在于探讨经导管二尖瓣修复术是否能够改善患者的预后。

来自美国和加拿大的78家中心参与了该研究,纳入的患者为中-严重二尖瓣反流或严重继发性二尖瓣反流(moderate-to-severe or severe secondary mitral regurgitation),尽管按照指南给予了最大剂量内科药物但是患者仍然有临床症状。纳入的患者随机给予经导管二尖瓣修复术+内科治疗(设备组)或仅内科治疗(对照组)。主要有效性终点为随访24h 个月内因心衰住院。主要安全性终点为12个月时未发生设备相关并发症。

共纳入了614例患者,302例为设备组,312例为对照组。设备组和对照组24个月内每年因心衰住院率分别为:35.8%和67.9%/患者-年(HR, 0.53; 95% CI, 0.40 to 0.70; P<0.001)。12个月无设备并发症的比例为96.6%(lower 95% confidence limit, 94.8%; P<0.001 for comparison with the performance goal)。设备组和对照组24个月内全因死亡分别为29.1%和46.1%(HR, 0.62; 95% CI, 0.46 to 0.82; P<0.001)。

最终作者认为对于合并中-严重或严重继发性二尖瓣反流的心衰患者,如果给予了最大推荐剂量药物治疗仍然存在症状时经导管二尖瓣修复术能够降低24个月因心衰住院的比例,降低全因死亡率(vs 仅内科治疗)。
