利用CRISPR基因编辑技术治疗癌症?

这项临床研究将从癌症患者体内提取出免疫系统的T细胞。接着,研究人员将利用CRISPR对T细胞进行基因修饰,并将基因修饰后的T细胞灌注回病人体内,这样它们将靶向摧毁肿瘤细胞。

NIH科学政策副主任Carrie Wolinetz在一篇博客帖子中披露了这一审查信息。宾夕法尼亚大学正在开发的这种癌症免疫疗法旨在靶向攻击骨髓瘤、黑色素瘤和肉瘤。

CRISPR技术是在不到四年前开发出来的,但是已正在冲向临床应用。在此之前,一家位于美国马萨诸塞州剑桥市的生物技术公司Editas医药公司(Editas Medicine)说,它打算在2017年开展一项利用CRISPR治疗一种罕见的眼部疾病的临床试验。

相比之下,这项新申请提出的利用CRISPR对人T细胞进行基因编辑治疗癌症的临床试验有可能更早开展。宾夕法尼亚大学并没有立即对寻求评论的要求作出回应,而且开展这项临床研究的时间也不能够确定。

据报道,宾夕法尼亚大学寻求NIH批准利用CRISPR技术剔除T细胞中的两个基因:一个基因是PD-1,它是人体免疫反应的一种关键性的关闭开关,抑制T细胞攻击肿瘤的能力,因此若没有这个基因,T细胞可能就能够让逃避免疫系统检测的肿瘤细胞现形,无处遁逃,当然剔除这个基因也可能给病人带来新的风险;另一个基因是一种T细胞受体编码基因,它能够调动人体的天然防御进行自我保护。

负责审查这一申请的RAC的使命是评估涉及基因疗法的研究的风险,而且也研究报道的死亡病例和副作用。经过基因修饰的T细胞也具有危险性,这是因为它们绕过体内正常的制衡,有可能让免疫系统攻击病人自身的组织。

Wolinetz说,“尽管在这个领域利用新的基因编辑技术有巨大的潜力改善人类健康,但是它不是没有令人担忧之处。”

宾夕法尼亚大学科学家Carl June博士开创性利用基因修饰的T细胞作为一种癌症治疗方法,它也被称作嵌合抗原受体T细胞免疫疗法(chimeric antigen receptor T-cell immunotherapy, CAR-T)。迄今为止,科学家们已使用过传统的基因改造方法让这些T细胞靶向肿瘤细胞表面上伸出来的特异性分子。但是相比于CRISPR,这种方法没有那么高效,而且靶向精确性经常比较差。

宾夕法尼亚大学这一申请的医学意义是不明确的,这是因为科学家们在此之前利用CRISPR/Cas9之外的基因编辑系统在人体开展过临床试验。

去年,一种不同的被称作转录激活子样效应因子核酸酶(TALEN)的基因组编辑技术,已被用来对T细胞进行基因修饰来治疗一名白血病英国儿童。这是TALEN首次被用来治疗一名病人。在由位于美国加州的生物技术公司桑加莫生物科技公司(Sangamo BioSciences)发起的一项临床试验中,第三种被称作锌指核酸酶(ZFN)的基因组编辑技术被用来治疗HIV/AIDS,并且在80多名病人中取得大有希望的结果。

但是,CRISPR技术被认为更加容易使用。Wolinetz说,“基因转移领域的科学家们对利用CRISPR/Cas9要比早期的基因编辑系统在更少的时间内以更少的成本修复或剔除参与多种人类疾病中的突变的潜力感到兴奋。”

大量的资金正在被投入这些相互竞争的基因组编辑技术中。就CRISPR技术而言,位于美国西雅图的朱诺治疗公司(Juno Therapeutics)向Editas医药公司承诺投入多达7.37亿美元利用它的CRISPR技术开展CAR-T研究,而诺华公司与基因编辑技术CRISPR/Cas9的领跑者Intellia治疗公司(Intellia Therapeutics)展开一项长达5年的投入2.8亿美元的研发合作计划,主要致力于加速发展CRISPR/Cas9技术在CAR-T细胞治疗和造血干细胞中的应用。
