Radiology:扩散加权成像特征能够间接反映前列腺癌肿瘤组织成分!

本研究共纳入了24例前列腺患者,并行磁共振(MR)成像,包括前列腺切除术前高b值DWI和DTI。在肿瘤内及非肿瘤的外周带(PZ)进行测量一下参数:单指数DWI、广延指数DWI、DKI、DTI中的表观扩散系数(ADC ME、ADC SE、ADC DKI、ADC DTI)、广延指数DWI中的反常扩散系数(α)、DKI中的峰度、DTI中的部分各向异性(FA)。利用配对T检验进行比较肿瘤区域及PZ的参数。利用Pearson相关分析分析肿瘤DWI与定量组织学参数(核,细胞质,细胞,基质,光质分数)的相关性。

结果为,前列腺癌和PZ间的所有DWI参数均有显着的差异(P < .012)。ADC ME、ADC SE、ADC DKI 与细胞质和细胞分数均成负性相关(r = 0.546--0.435; P < .034),与基质成分具有正性相关性(r = 0.619-0.669; P < .001)。ADC DTI、FA仅与基质成分具有相关性(r = 0.512、-0.413; P < .045)。α与组织学参数均无相关性,而峰度与组织学参数具有显着的相关性(与细胞质分数、细胞分数及基质分数的r分别为0.487, 0.485, -0.422; P < .040)

本研究表明,在前列腺癌中,高级DWI参数与组织病理学组织成分具有显着相关性。
