ANN Oncol:原发部位未知肿瘤的临床和分子特征

以往数据显示,原发部位未知的肿瘤(CUP)预后很差,总生存时间不足1年。治疗方式通常为根据病理特征和转移情况进行化疗。ANN ONCOL近期发表了一篇文章,分析CUP患者临床和病理情况,寻找这类患者中常出现的分子突变,研究分子检测如何改变治疗决策以及二代测序在CUP患者评估和治疗中的作用。

作者收集了2年内在其中心进行治疗的CUP患者,收集患者人口学信息,临床结局,病理情况,肿瘤组织二代测序结果,靶向治疗情况以及临床试验纳入情况。研究共纳入2014年1月1日到2016年6月30日期间进行评估的CUP患者333例。这些患者中,150例接受了二代基因测序。这一队列的中位总生存时间为13个月。150例患者中有45例具有潜在的基因改变靶点,15例接受过靶向治疗。150例患者中有21例存在显性突变,大部分与外源性诱变剂如紫外线和烟草有关。

文章最后认为,CUP患者之间异质性很明显,具有很多潜在的突变靶点。二代基因测序可以为CUP患者提供接受个性化治疗并获益的机会
