炎症过程可能在血管疾病的发病中发挥重要作用,血液循环中炎症标志物与冠心病(CHD)的相关性引起了大家的兴趣。以往的流行病学研究主要报道了“下游”炎症标志物——如C反应蛋白(CRP)和纤维蛋白原等——与冠心病事件之间的联系。
 
然而,人类遗传证据并不太支持这些肝源性因子与冠心病事件有因果关系。相比之下,“上游”炎症标志物,如促炎细胞因子,更有可能直接与冠心病有关,因为他们控制炎症级联反应。这些标志物与冠心病关系的大多数流行病学证据是关于白细胞介素-6(IL-6)。之前的研究工作表明,与一些主要已确定的危险因素一样,长期可溶性IL-6水平与冠心病风险正相关,IL-6信号通路与冠心病的因果关系已经被人类遗传支持证据。这些研究结果最近引发了更多关注,并启动了使用各种抗炎药物进行心血管疾病(CVD)的二级预防的大规模的3期临床试验。

研究概要
 
本研究基于轻度炎症可能在冠心病的发病中发挥作用、促炎因子控制炎症级联反应的假设,旨在评价五个促炎细胞因子和冠心病风险的关联,是一项前瞻性研究,包括对前瞻性研究的荟萃分析。
 
研究者在丹麦预防和健康中心(RCPH)进行了一项基于丹麦人群的前瞻性队列研究,共有1514名参与者,833例发生冠心病事件。参与者年龄30-70岁,平均随访12年。在基线时测定白细胞介素-6(IL - 6)、IL-18、基质金属蛋白酶-9(MMP-9)、可溶性CD40配体(sCD40L)和肿瘤坏死因子-α(TNF-α)水平。
 
在调整年龄和性别后,与基线水平对数转换每增加1个标准差,CHD的风险比率(HR)分别为:
 
IL-6:1.37(95%CI 1.21-1.54)
IL-18:1.26(95%CI 1.11-1.44)
MMP-9:1.30(95%CI 1.16-1.46)

研究者对29项基于人群的前瞻性研究进行了Meta分析。炎症因子指标每增加1个标准差,调整后的非致死性心肌梗死或CHD相对风险(RR)为:
 
IL-6:1.25(95%CI 1.19-1.32)
IL-18:1.13(95%CI 1.05-1.20)
MMP-9:1.07(95%CI 0.97-1.19)
sCD40L:1.07(95%CI 0.95-1.21)
TNF-α:1.17(95%CI 1.09-1.25)
 
数据表明,不同的促炎细胞因子都与冠心病风险相关,这种风险独立于传统危险因素之外,呈接近对数线性的相关。这些发现支持血管疾病的炎症假说,但还需要进一步的研究来评估因果关系。
