胃功能检查—胃泌素-17、胃蛋白酶原Ⅰ、胃蛋白酶原Ⅱ异常,怎么一回事?

患者男性,39岁,今年1月8日例行检查时发现胃泌素-17明显低下(0.04pmol/L,参考值1.0~15.0pmol/L),胃蛋白酶原Ⅰ、胃蛋白酶原Ⅱ都低下而两者比例增高,请问这有什么问题吗?

患者男性,39岁,今年1月8日例行检查时发现泌素-17明显低下(0.04pmol/L,参考值1.0~15.0pmol/L),蛋白酶原Ⅰ、胃蛋白酶原Ⅱ都低下而两者比例增高,请问这有什么问题吗?

近两天感觉上腹饱胀,只吃了一点点东西。

而2018年5月胃泌素-17及胃蛋白酶原检测报告显示数值正常。当时同时做胃镜显示:慢性非萎缩性胃炎、十二指肠球炎。

解析

主要是胃功能四项检查异常,考虑:慢性萎缩性胃炎,胃窦黏膜萎缩。

胃泌素-17是由胃窦部及十二指肠近端黏膜中G细胞分泌的一种胃肠激素,主要刺激胃壁细胞分泌盐酸。

胃泌素-17测定可反映胃窦G细胞数量和功能状态,有助于判断胃窦病变的轻重和胃酸分泌情况;有助于判断萎缩是否存在及其分布部位和程度;有助于胃癌诊断

多数学者认为,空腹血清胃泌素-17小于1pmol/L可作为胃窦萎缩性胃炎的界限值。

胃蛋白酶原(PG)水平反映胃黏膜的功能状态。当胃黏膜出现萎缩时,PGⅠ和PGⅡ水平下降,PGⅠ水平下降更明显,因而PGⅠ/Ⅱ比值随之降低。

PG测定有助于判断萎缩的范围。

(1)胃体萎缩者,PGⅠ、PGⅠ/Ⅱ比值降低,血清胃泌素-17水平升高;

(2)胃窦萎缩者,血清胃泌素-17水平降低,PGⅠ、PGⅠ/Ⅱ比值正常;

(3)全胃萎缩者则两者均降低。

通常使用PGⅠ水平≤70 μg/L,且PGⅠ/Ⅱ比值≤3.0,作为萎缩性胃炎的诊断临界值,并以此作为胃癌高危人群筛查的标准,在欧洲和日本广泛用于胃癌风险的筛查;国内胃癌高发区筛查常采用PGⅠ水平≤70 μg/L且PGⅠ/Ⅱ≤7.0的标准。  

血清PG检测有助于胃癌高危人群的风险分层,PG检测诊断萎缩者,以及PG检测虽诊断萎缩阴性、但PGⅠ/Ⅱ比值较低者,有较高的胃癌风险,应进一步进行胃镜检查。

胃功能检查:胃泌素-17下降,PGⅠ和PGⅡ水平下降,但PGⅠ/Ⅱ比值没有下降,应考虑:胃窦黏膜萎缩。

患者在2018年5月胃镜检查报告慢性非萎缩性胃炎,但病理检查示胃窦大弯侧萎缩性胃炎(轻度),胃窦体交界大弯侧非萎缩,说明在2018年5月时患者已经有了萎缩性胃炎。因为胃镜检查只有在胃黏膜萎缩比较明显时才能确定,与病理检查可以不一致,萎缩性胃炎的诊断以病理为准。但从现在胃功能来看,PGⅠ/Ⅱ比值没有下降,胃癌风险不大,建议两年检查一次胃镜。

患者在2018年5月已有胃窦萎缩,但当时胃功能检查正常,可能的原因:

1. 当时萎缩轻,现在胃窦萎缩加重了。

2. 胃功能检查受一些因素影响,如餐后可以升高,与服胃药有关等等。因此要求:血清样本采集前2周患者停服抑酸药,前1天停服胃黏膜保护剂,采集前10 小时保持空腹,禁烟酒。要求患者晨起空腹采集静脉血。

病理检查显示幽门螺杆菌阳性,不知道患者有没有根除幽门螺杆菌。幽门螺杆菌是胃黏膜炎症的重要原因,可引起、加重胃黏膜萎缩。另外,生活不规律、饮食不当、某些药物等因素也可加重胃病。

萎缩性胃炎,首先根除幽门螺杆菌,可以逆转轻度萎缩,防止病情加重。建议患者停止胃药后去做一次幽门螺杆菌呼气试验,若阳性必须根除。

另外萎缩性胃炎的治疗主要是胃黏膜保护剂,一些中成药也具有黏膜保护等的综合性治疗作用。
