尿酸是人体重要的内源性抗氧化物质,既往研究发现,卒中患者血清尿酸水平高预示预后以及溶栓治疗疗效较好。但是,卒中后尿酸水平通常会迅速降低,因而,补充外源性尿酸可能是提高急性期溶栓疗效及改善卒中预后的新策略。

URICO-ICTUS研究是一个在西班牙开展的关于阿替谱酶联合尿酸治疗急性期卒中的随机、双盲、安慰剂对照IIb/III期临床试验。试验结果于近期发表于《柳叶刀神经病学杂志》上。

该研究自2011年6月1日至2013年4月30日之间一共入组411例急性缺血性卒中患者。评价内容为发病4.5小时内接受阿替普酶静脉溶栓联合1000mg剂量尿酸,或阿替普酶联合安慰剂治疗90天后预后良好的患者比例。研究还评估了两种治疗手段的不良反应情况。

结果显示,阿替普酶联合尿酸组和阿替普酶联合安慰剂组90天时预后良好的患者例数分别为83(39%)例和66(33%)例。两组之间在患者死亡率、症状性颅内出血发生率以及其它药物相关不良反应方面亦无统计学差异。

该结果提示,尽管在溶栓治疗时合并使用尿酸并不增加溶栓并发症及其它不良反应的发生,但由于合并用药并未使患者预后明显改善,因而,临床上给予卒中患者实施阿替普酶联合尿酸的治疗是不合适的。然而,仍有相当一部分患者由于时间窗的限制无法接受溶栓治疗,对于该部分患者,补充外源性的尿酸是否能够带来益处,还应进一步探索证明。
