Gut:髓源性抑制细胞抑制在慢性急性发作性肝功能衰竭患者中的应用

慢性急性发作性肝功能衰竭(ACLF) 患者免疫麻痹可导致患者感染易感性和死亡率增加。研究发现免疫抑制单核CD14+HLA-DR−骨髓衍生抑制细胞(M-MDSCs) 会抑制免疫介导疾病的抗菌反应。近日研究人员就M-MDSC在ACLF患者中的应用前景开展研究。

研究招募了41名ACLF患者,25名健康对照者、22名肝硬化以及30名急性肝功能衰竭患者,对其分别进行M-MDSC分析,抑制T细胞活化研究以及先天免疫功能的评价(Toll样受体刺激及噬菌作用分析)。

研及发现,ACLF患者体循环中CD14+CD15−CD11b+HLA-DR− M-MDSCs数量显著上升。M-MDSCs显示出免疫抑制特性,显著减少T细胞增殖(P = 0.01),降低肿瘤坏死因子α/白细胞介素-6对Toll样受体(TLR)刺激的响应(全部p<0.01),减少大肠杆菌的细菌吞噬。持续性的HLA-DR低表达与二次感染和28天死亡率增加相关。体外研究表明,对TLR-2和TLR-4的反复刺激可导致M-MDSC表达增加,体内研究发现,TLR-3激动可激活HLA-DR的表达,激活先天免疫功能。
