JAMA Neurol：深部脑刺激治疗杜尔雷斯综合征

研究发现，深部脑刺激可有效改善杜尔雷斯综合征患者抽搐症状，但严重不良事件的发生风险也有所提高

大量的研究表明深部脑刺激(DBS)具有治疗杜尔雷斯综合征的应用前景。近日研究人员开展跨国队列研究考察DBS治疗杜尔雷斯综合征的疗效和安全性。

研究招募了185名药物耐受的杜尔雷斯综合征患者，患者来自10个国家的31个医疗中心。患者随机接受中央正中丘脑区DBS（57.1%）、内苍白球前DBS（25.2%）、内苍白球后DBS（15.2%）或者内囊前翼DBS（2.5%）。研究的主要终点为耶鲁大学抽搐严重程度评分和不良事件。

参与者中，女性37人，手术时平均年龄29.1岁。64.2%的患者有强迫症症状，21.6%的患者有自残史。DBS治疗1年后，患者的抽搐严重程度评分由基线时的75.01分下降至41.19分；运动抽搐得分由基线时的21.00分下降至12.97分；声音音质评分由基线时的16.82分下降至9.63分。35.4%的患者出现不良事件，其中2人出现颅内出血，4人发生5起感染事件，1人出现设备问题。最常见的刺激引起的不良反应为构音障碍(10例，6.3%)和麻痹(13例，8.2%)。

研究发现，深部脑刺激可有效改善杜尔雷斯综合征患者抽搐症状，但严重不良事件的发生风险也有所提高。
