Pembrolizumab(MK-3475)是一种人源化的IgG4，与PD-1受体有高度亲和力的抗PD-1抗体，能阻断PD-L1和PD-L2的双重配体。

本项Ⅰb期研究显示，Pembrolizumab在难治性、PD-L1阳性、进展期三阴(ER-/PR-/HER2-)乳腺癌患者中显示出可接受的安全性和耐受性。总有效率(ORR)达18.5%。Pembrolizumab的效应持久，未达到中位反应持续时间(15~40周以上)，治疗有效的5例研究对象中3例的反应持续时间≥11个月。

基于可接受的安全性和耐受性，以及可观的抗肿瘤活性的支持，研究者将对pembrolizumab在进展期三阴性乳腺癌患者中展开进一步的研究，Ⅱ期研究预计将于2015年上半年进行。

三阴乳腺癌是目前被公认为临床治疗效果最差，争议最多的一类亚型。PD1/PDL1免疫疗法是当前备受瞩目的新一代免疫疗法，旨在利用人体自身的免疫系统抵御肿瘤，通过阻断PD1-PD-L1信号通路诱导凋亡，具有治疗多种类型肿瘤潜力。近年来研究结果表明不仅彻底改变了晚期黑色素瘤的治疗，在其他实体瘤如非小细胞肺癌，膀胱癌，头颈癌也显示一定前景。被认为是靶向治疗后癌症治疗的革命。未来免疫调节点药物互相联合或与其他抗癌药物的联合也将成为研究的热点。期待未来带给三阴乳腺癌患者一种有效治疗手段。
