2022年1月，日本心血管介入治疗协会(CVIT)更新发布了急性心肌梗死（AMI）直接经皮冠状动脉介入治疗（PCI）共识2022更新版。直接经皮冠状动脉介入治疗对于降低ST段抬高型心肌梗死患者死亡率有重大贡献。本文是对2018年共识的更新，主要聚焦PCI实施过程方面。直接经皮冠状动脉介入治疗(PCI)显著降低了ST段抬高型心肌梗死(STEMI)患者的死亡率，甚至在心源性休克时也是如此，现在是大多数日本机构的标准护理。日本心血管介入与治疗学会(CVIT)的初次冠状动脉介入治疗特别工作组于2018年提出了一份关于急性心肌梗死(AMI)治疗的专家共识文件，重点是直接经皮冠状动脉介入治疗的程序方面。欧洲心脏病学会(ESC)于2017年和2020年发布了最新的急性心肌梗死治疗指南。STEMI患者指南的主要变化包括：(1)推荐使用放射状支架和药物洗脱支架(DES)而不是裸金属支架(BMS)，(2)出院前(无论是立即还是分期)完全血运重建现在被认为是IIa级建议。2020年，更新的非ST段抬高心肌梗死(NSTEMI)患者指南做出了如下改变：(1)对于NSTEMI作为I类适应症的患者，建议在24小时内采取早期有创策略；(2)对于无心源性休克的NSTEMI患者，建议进行完全血运重建；(3)对于短期三联抗血栓治疗后的心房颤动患者，建议进行双重抗血栓治疗(例如，DOAC和单一口服抗血小板药物，最好是氯吡格雷)，并在6至12个月后停用抗血小板药物。此外，在几个已经开始显示安全性和有效性的试验中，已经研究了一种在经皮冠状动脉介入治疗后不使用阿司匹林的策略。
