盘点:肝癌近期重要研究汇总

【1】pIgR/p-Yes 亚群分型是肝癌患者生存的预后标志物

pIgR 通常感受炎性微环境调控,介导细胞内多聚免疫球蛋白 IgA 和 IgM 的极性转运,是粘膜上皮组织最为重要的一个连接天然免疫和获得性免疫的免疫球蛋白受体。中国科学院上海药物研究所的科研人员在前期工作中首次发现了 pIgR 感受炎性微环境上调,通过募集活化 Smad 复合物,显着诱导肝癌细胞的上皮细胞间充质转化(EMT),促进肝癌的早期复发与转移。该研究挑战了对 pIgR 传统功能认识的局限,为免疫球蛋白受体的两面性提供了重要范例。pIgR 采用生理条件下免疫转运相关通路介导肿瘤细胞恶性生长,为 pIgR 免疫背叛的深入理解奠定了基础。同时,研究工作也有效拓展了 Dasatinib、MEK 抑制剂的新敏感群体,对该类药物的临床应用具有重要的指导意义。

【2】氨基酸转运蛋白在肝癌发生过程中的重要调节作用

近日,清华大学药学院陈立功课题组及其合作者阐述了氨基酸转运蛋白在肝癌发生过程受原癌基因c-Myc调控进而激活mTORC1信号通路诱导肝癌形成。研究人员发现mTORC1信号通路的激活是c-Myc诱导肝癌形成的必要条件, mTOR 下游效应器真核细胞翻译启始因子4E 结合蛋白1 4EBP1 / eIF4E和核糖体蛋白S6 激酶1 p70S6K 共同调节细胞增殖。氨基酸作为细胞生长和增殖的必要条件,对mTORC1正常发挥功能起着至关作用,转录组学数据分析表明c-Myc能够上调多种氨基酸转运蛋白的表达,如SLC1A5和SLC7A6,它们属于膜转运蛋白的溶质载体(SLC)家族,这些转运蛋白在各种生理和病理过程中具有关键的作用。代谢组学分析表明c-Myc直接上调SLC1A5和SLC7A6,导致肝癌细胞对氨基酸的吸收增加进而激活mTORC1信号通路。在人类HCC标本中,c-Myc的水平和SLC1A5 / SLC7A6表达之间有很大的相关性。总之,这些数据表明在肝癌的治疗过程中,除了靶向mTOR信号通路以外,SLC氨基酸转运蛋白家族也渐渐成为有效的药物的靶标,成为治疗HCC有效选择。

【3】剪接蛋白SRSF2在肝癌发生发展中的调控作用


日前,国际学术期刊Cancer Research在线发表了上海生科院(健康院)冯英组的最新研究进展,揭示剪接蛋白SRSF2通过调控与肿瘤相关基因的选择性剪接,从而促进肝癌的发生发展。研究发现,SRSF2对肝癌细胞的生长有明显的促进作用,并且其功能的发挥大部分依赖于其对下游多个基因的剪接调控。SRSF2调控靶基因GCH1和STK39等的选择性剪接,导致促进肝癌细胞生长的剪接异构体表达增加,后者与肝癌的恶性程度相关联。该研究表明剪接蛋白SRSF2通过调控基因如GCH1,STK39等的选择性剪接,从而在肝癌的发生发展中发挥重要功能,这一工作不仅有助于深入了解肝癌的发病机制,并且也为将来的诊断和治疗提供了新的靶点和思路。

【4】乐伐替尼有望成为中晚期肝癌治疗新标准

索拉非尼开启了晚期HCC分子靶向治疗的大门,改善了全球范围内部分肝癌患者的预后,但远远不能满足广大患者的治疗需求。一直以来肝癌一线分子靶向治疗药物只有索拉非尼,一系列挑战索拉非尼一线治疗地位的分子靶向药物如舒尼替尼、布立尼布、利尼伐尼、多韦替尼、尼达尼布,均以失败告终。令人感到振奋的是在今年ASCO年会上,美国当地时间6月4日8:12,报告了乐伐替尼头对头索拉非尼一线治疗肝癌国际III期临床研究REFLECT研究的结果,乐伐替尼在主要终点总生存OS达到预期结果,次要终点无进展生存(PFS)、进展时间(TTP)和客观反应率(ORR)全面超越索拉非尼。REFLECT研究是一项随机、开放式、非劣效(NI)研究,研究证实乐伐替尼一线治疗晚期肝癌的效果不劣于索拉非尼。

【5】IL-18基因多态性对乙肝后肝硬化和肝癌易感性的影响

IL-18基因多态性影响了IL-18基因的转录活性,并且与多种疾病相关。目前IL-18基因多态性与乙肝病毒相关肝脏疾病这两者之间的关系还没有统一的观点。新的一项研究发现,rs187238 CG基因型的人群发生肝硬化的风险更低。与健康组CC和CG基因型相比,GG基因型的乙肝病毒携带者发生肝癌的风险更高。rs187238 GG基因型增加了的乙肝病毒携带者进展为肝硬化的风险。表明,IL-18 rs187238 GG基因型会增加健康人群发生肝癌的风险,也会增加乙肝病毒携带者发生肝硬化的风险。

【6】DNA损伤检查点G2/M在肝癌早期发生起重要作用

发表于Oncotarget的一篇文章中,研究人员分析研究了原发性肝癌 的早期分子机制。结果发现,细胞周期中DNA损伤检查点的调节是HCC早期发生的主要途径。通过使用Kaplan-Meier生存分析发现,包括CCNB1、CDC25B、XPO1、GMPS、KPNA2和MELK在内的几种基因的高表达与HCC患者的高风险,预后差和总生存期短相关。G2/M检查点在早期HCC中起重要作用,参与该过程的基因具有作为 诊断和预后生物标志物的潜力,有待进一步研究。
