BMJ:接种新冠肺炎疫苗不会增加免疫介导性神经系统疾病风险

Covid-19疫苗不会增加贝尔麻痹、脑脊髓炎、格林-巴雷综合征和横贯性脊髓炎等免疫介导的神经系统疾病风险,相反SARS-CoV-2感染患者中,上述疾病的风险增加。

截至2022年1月13日,新冠肺炎大流行已导致全球550多万人死亡。迄今为止,已有五种针对SARS-CoV-2的疫苗获得了欧洲药品管理局的许可,其中包括两种mRNA疫苗:BNT162b2和mRNA-1273-1273、两种病毒载体疫苗:ChAdOx1nCoV-19和Ad.26.COV2.S以及一种带佐剂的重组刺突蛋白纳米颗粒疫苗:NVX-CoV2373。近日研究人员考察了Covid-19疫苗、SARS-CoV-2感染免疫介导的神经疾病风险之间的关系。


研究人员考察了英国和西班牙居民保健记录数据库数据,8330497名接受了至少一剂Covid-19疫苗(ChAdOx1nCoV-19、BNT162b2、mRNA-1273或Ad.26.COV2.S)个体参与研究,样本还包括735870 名未接种疫苗的阳性感染个体和14330080名一般个体。研究的主要终点为在疫苗接种后21天或SARS-CoV-2检测结果呈阳性后90天贝尔麻痹、脑脊髓炎、格林-巴利综合征和横贯性脊髓炎的发生率。
接种过疫苗的人群中, 376535人接种ChAdOx1 nCoV-19,3588318人接种了BNT162b2,244913人接种mRNA-1273,120731人接种Ad26.CoV.2。与一般人群相比,疫苗接种人群的贝尔麻痹、脑脊髓炎、格林-巴利综合征的发病率相近,但SARS-CoV-2感染后上述疾病发病率增加:贝尔麻痹发病风险为1.33,脑脊髓炎风险为6.89,格林-巴雷综合征风险为3.53,横贯性脊髓炎由于病例较少未进行系统分析。
