何为维生素K

维生素K由丹麦营养生物学家亨里克 达姆(Henrik Dam)于20世纪30年代首次发现,初期对于维生素K作用的研究主要集中于其凝血功能。1975年佩蒂弗(Pettifor)等首次提出维生素K族化合物参与人体骨代谢的推断。

此后有关维生素K治疗骨质疏松症、预防老年性骨折的基础研究及流行病学研究大量涌现,维生素K在骨代谢过程中的独特功效及重要作用不断被揭示。

维生素K是一族具有叶绿醌强抗氧化生物活性的甲萘醌系列衍生物。天然存在的维生素K1和K2是目前参与人体代谢研究及应用最多的两类化合物:①维生素K1或称叶绿醌,是膳食维生素K的主要来源,在绿叶蔬菜和一些植物油中含量丰富,例如羽衣甘蓝、菠菜、西兰花、橄榄油等;②维生素K2(MK-n),MK-n主要由肠道细菌合成,其中MK-4还能通过膳食维生素K1转化获得。食源性MK-n可从肉、鱼、蛋、奶、肝脏、牛油和发酵大豆中获取。

维生素K1和K2对机体宏观骨密度都有贡献,但维生素K2是机体内维生素K的主要活性形式。维生素K1须转化为K2方可发挥其生物学作用,由于转化率较低,故维生素K2更受重视,相关研究亦更为深入。

维生素K与骨质疏松症改善骨密度,治疗骨质疏松症

韩国诸(JeSH)等研究显示联合应用维生素K2、维生素D和钙可减少未羧化骨钙素的血药浓度,同时可增加60岁以上绝经后妇女的骨密度。平尾(HiraoM)等研究显示维生素K2和阿仑膦酸钠合用可增加绝经后骨质疏松症女性的骨钙素羧化和骨密度。布拉姆(Braam)对155名绝经后女性随访3年的研究发现,补充维生素K2组髋部骨密度下降幅度较安慰剂组低1.7%,较补充钙、镁、锌和维生素D组低1.3%,且维生素K2与维生素D在增加骨密度上具有协同作用。以上研究表明维生素K2可与维生素D、钙等合用,用于改善骨密度,治疗骨质疏松症。

预防骨折的发生

研究显示,维生素K缺乏可增加年龄相关性骨丢失及骨质疏松性骨折的患病风险。

骨质疏松症患者每日补充维生素K2后2年,腰椎骨密度显著高于未治疗的对照组,椎体骨折发生率降低达64%,其他观察部位(前臂、股骨颈及其他)的骨折发生率降低56%。张(CheungAM)等对440名绝经后骨质疏松症患者进行的一项为期2年的随机对照研究显示,补充维生素K后2~4年,不能防止与年龄相关的骨密度下降,但可能减少绝经后女性骨质疏松性骨折及癌症的发病率。

可见,维生素K具有骨折预防作用。

维生素K的膳食推荐摄入量

随着对维生素K生物学功效的研究,各国开始对健康人群维生素K每日膳食摄入量进行界定。2005年日本膳食参考摄入量(DRIs)指出:≥30岁女性维生素K的适宜摄入量(AI)为65μg/d。中国营养学会制定的《中国居民膳食营养素参考摄入量(2013版)》中指出,中国18岁以上人群,维生素K的AI应达80μg/d。

对骨质疏松症患者的维生素K摄入问题,在多个国家及学术组织制定的相关指南中亦有体现,例如中国老年学学会骨质疏松委员会制定的《中国人群骨质疏松症防治指南手册(2013版)》及美国临床内分泌医师学会(AACE)制定的《绝经后骨质疏松诊治指南》等。有学者提出,对于骨质疏松症患者及高危人群,维持骨健康需要更高的维生素K摄入量。《中国人群骨质疏松症防治指南手册(2013版)》指出健康成人每日须补充的维生素K2约50~120μg,而骨质疏松及骨骼亚健康人群的维生素K2补充量则须适当增加。宾克利(Binkley)等报道,围绝经期骨质疏松症患者要使循环中骨钙素充分γ羧基化,每日维生素K摄入量应达1000μg,该数据亦被收录至2010年AACE指南中。

常见食物中维生素K的含量


从膳食补充维生素K角度出发,天然维生素K1广泛存在于绿色植物及植物油中,维生素K2则主要由肠道细菌合成,少量从食物中摄取(表)。膳食摄取维生素K主要通过蔬菜(菠菜、甘蓝等光合作用强的深色蔬菜)及植物油。

但亦有文献支持,维生素K2是人体组织维生素K的真正活性形式,在人体内主要由肠道细菌合成。中年以后由于机体机能衰退,肠道维生素K2合成能力急剧下降,加之中国人群膳食结构的原因,难以从饮食中获得较充足的维生素K2,因此中老年人尤其是绝经后女性需要适时补充一定量的维生素K2。

关于维生素D

维生素D对构建与维持健康骨骼发挥两个关键作用:帮助人体从肠道中吸收钙;确保骨骼正常的更新与骨矿质化。

维生素D帮助改善肌肉力量与平衡,降低跌倒风险。

当皮肤接受阳光的紫外线(UV-B)照射后会产生维生素D。

阳光不一定总会促成维生素D的合成,季节和纬度、使用防晒霜、城市烟雾、皮肤色素沉着、个人年龄等因素都会影响皮肤透过阳光照射所产生的维生素D量。

儿童体内严重缺乏维生素D会导致生长迟缓和骨骼畸形,即佝偻病。成年人缺乏维生素D会导致软骨病,这就是由于骨骼矿质化不良而造成的骨骼软化。

维生素D水平低值得引起全世界的关注,因为这预示着人们患骨质疏松症的风险会提高。

维生素D的食物来源包括:油性鱼类(例如鲑鱼、鲭鱼和沙丁鱼)、蛋黄和动物肝脏。在一些国家,牛奶、人造黄油和早餐麦片都添加了维生素D。

维生素D的建议摄取量随着年龄层和人生阶段而改变,需求量随着年龄而增加。

人们晒太阳应该在每天上午10点以前或者下午2点以后阳光不是最强烈的时段,不要擦防晒霜,晒太阳10~20分钟,但注意不要晒伤。
