J Thromb Haemost:癌症相关的内脏静脉血栓形成的临床病史

尽管基线特征和抗凝治疗存在一些差异,但SVT的癌症患者和常规VTE的癌症患者发生复发性血栓形成和出血的风险似乎相似。有必要进行进一步的前瞻性研究来证实这些结果。

癌症是内脏静脉血栓形成(SVT)和常见部位静脉血栓栓塞(VTE)的危险因素。近日,血栓和凝血疾病权威杂志Journal of Thrombosis and Haemostasis上发表了一篇研究文章,该研究旨在比较癌症相关SVT和常规部位VTE患者的特征和结局。

研究人员在2008年5月至2012年1月之间从国际前瞻性登记研究中招募了实体癌和SVT患者。比较队列以1:1比例纳入了实体癌患者和接受过VTE常规治疗的患者,他们在2019年12月前至少接受了12个月的随访,或在12个月的随访期间发生了其中一个结局。在12个月的随访中,研究人员评估了复发性VTE、大出血和全因死亡率。

264名患者(每组132人)入组,SVT患者不太可能发生转移性疾病(36.1% vs. 72.5%)或在血栓形成诊断时接受癌症治疗(29.6% vs. 64.9%)。在SVT患者队列中最常见的癌症类型是肝胆和胰腺癌,在VTE患者队列中最常见的是胃肠道癌症。接受抗凝治疗的SVT患者更少(68.9% vs. 99.2%),治疗时间更短(6.0 vs. 11.0个月)。在SVT患者队列中,大出血的累积发生率(2.3% vs. 4.7%)显著更低,而复发性血栓形成(4.7% vs. 5.5%)和全因死亡率(41.7%对39.4%)在两个队列中相当。

由此可见,尽管基线特征和抗凝治疗存在一些差异,但SVT的癌症患者和常规VTE的癌症患者发生复发性血栓形成和出血的风险似乎相似。有必要进行进一步的前瞻性研究来证实这些结果。
