叶酸和维生素B12不足被认为与抑郁风险增加有关。本文旨在评估与安慰剂相比,通过叶酸或维生素B12治疗是否可降低风险人群的抑郁评分表得分,并缓解和防抑郁症临床症状的发作。

我们在PubMed、PsychInfo、Embase及Cochrane数据库中,通过关键词(vitamin B12 or vitamin B9 or folate or folic acid or cobalamin or cyanocobalamin) and (depression or depressive disorder or depressive symptoms) and (randomized controlled trial or RCT)进行相关研究的检索,时间截止到2014年6月6日。两位评审人员对所检索到的研究进行评估并提取原始数据。数据分析和管理通过Review Manager 5来评估。

我们检索到269项研究(包含RCTs研究52篇),收纳了11项符合评估标准的研究。我们发现短期维生素治疗(几天至几周)不会改善接受抗抑郁药物治疗的患者(5项研究,标准平均差=-0.12,95% CI=-0.45,0.22),但延长维生素使用时间(数周至数年)可能会降低抑郁复发(1项研究,OR=0.33,95% CI=0.12,0.94)和临床症状发作风险(2项研究,风险率= 0.65, 95% CI = 0.43, 0.98)。

结果表明,在很短的时间内使用叶酸和维生素B12治疗不能缓解抑郁症状,但是在特定人群中可能有助于长期管理。关于叶酸,维生素B12可提高抗抑郁药物应答相关研究依然很少,而且研究之间的异质性高。
