慢加急性肝衰竭(ACLF)是指在慢性肝病基础上，由急性打击诱发的肝功能急剧恶化，可在短时间内出现多器官功能衰竭，目前尚缺乏理想的治疗手段。早期准确预测患者的临床转归，有助于优化治疗方案，提高患者生存率。回顾了近年来有关ACLF预后模型的研究，简述各种评分系统的特点及其预测能力，旨在指导ACLF的治疗选择及预后判断。

慢加急性肝衰竭(ACLF)是指在慢性肝病基础上，由急性打击诱发的肝功能急剧恶化，可在短时间内出现多器官功能衰竭，目前尚缺乏理想的治疗手段。早期准确预测患者的临床转归，有助于优化治疗方案，提高患者生存率。回顾了近年来有关ACLF预后模型的研究，简述各种评分系统的特点及其预测能力，旨在指导ACLF的治疗选择及预后判断。



目前临床上常用的预后模型尚存在预测准确性不足、计算繁琐等问题。COSSH-ACLF模型基于大样本、多中心的前瞻性临床研究建立，且研究人群均为慢性乙型肝炎患者，可能成为判断我国ACLF患者预后的最佳模型，但其预测价值仍需进一步的临床研究来证实。如何建立计算简单、预测准确且能够动态评估患者病情变化的预后模型，仍是今后ACLF预后研究面临的主要挑战。
