众所周知,C26:0溶血磷脂酰胆碱(LPC)升高是x -肾上腺脑白质营养不良(X-ALD)的诊断指标。本研究的目的是开发一种流动注射电离串联质谱(FIA-MS/MS)方法来估计干血滴(DBS)中的LPCs (C20:0-C26:0-LPCs),并确定该方法用于高通量筛查X-ALD的敏感性和特异性。

众所周知,C26:0溶血磷脂酰胆碱(LPC)升高是x -肾上腺脑白质营养不良(X-ALD)诊断指标。本研究的目的是开发一种流动注射电离串联质谱(FIA-MS/MS)方法来估计干血滴(DBS)中的LPCs (C20:0-C26:0-LPCs),并确定该方法用于高通量筛查X-ALD的敏感性和特异性。

研究人员在一个96孔从3.2毫米DBS板萃取LPCSC200-C260),并进行同位素标记的内标(C260-D4-LPC),采用三重四极串联质谱仪在FIA-MS / MS电喷雾离子化测得的(ESI)阳性,多反应监测(MRM)模式检测。同时确定FIA-MS / MS方法对X-ALD的筛查的灵敏度和特异性测定,并对FIA-MS / MS方法与LC-MS / MS方法用于估计LPC浓度进行比较

研究显示C26:0C24:0-LPCs升高对X-ALD的鉴别具有100%的敏感性。然而,C26:0C24:0-LPCs的特异性仅为78.33%98.33%C22:0C20:0 LPCs的敏感性分别为89.29%78.33%和特异性分别为67.86%73.33%FIA-MS/MS方法与LC-MS/MS方法具有良好的一致性。

研究表明,用FIA-MS/MS方法估计DBSC26:0C24:0-LPCs适用于新生儿X-ALD的一级筛查。筛查阳性病例需要二级验证性试验。
