以细胞为基础的疗法可以治疗传统疗法无法治疗的疾病。其中基于间充质干细胞(MSCs)的治疗方法特别有前景,因为它们是具有多潜能的自体细胞的来源,并且可以相对容易地从患者身上收集。骨髓间充质干细胞也可以自我更新,具有免疫抑制特性,使其成为自体细胞治疗的潜在候选细胞。在血管治疗方面,骨髓间充质干细胞已被探索用于治疗心肌梗死和周围性缺血。然而,这些试验并没有显示出对患者的长期益处。MSCs有几个方面限制了其在细胞治疗中的应用。在常规培养条件下,骨髓间充质干细胞在扩增后失去分化潜能,治疗性能降低。此外,分离的MSC群体具有高度的异质性,,骨髓间充质干细胞的治疗潜力会因患者的健康状况而改变。这是一个主要限制因素。高龄、肥胖、糖尿病和其他慢性疾病患者的骨髓间充质干细胞再生能力降低。尽管患者间骨髓间充质干细胞的行为存在差异,再生能力降低,但研究人员对骨髓间充质干细胞有效和可靠的替代策略有着浓厚的兴趣。

骨髓间充质干细胞向血管细胞的分化对于许多临床应用是非常有利的,包括缺血的治疗和组织工程血管移植。对于诱导骨髓间充质干细胞内皮分化的条件,还没有共识。生物物理力,包括剪切应力和机械拉伸,已被用于MSCs诱导。一些研究表明,剪切应力和治疗(如VEGF)可导致MSCs中内皮标志物的表达。目前尚不清楚在MSCs中获得内皮细胞样表型的最佳条件。此外,目前尚不清楚增强内皮或血管表型是否也能提高MSC治疗的有效性。在这篇文章中,使用一个高通量的系统对培养的细胞施加机械拉伸来检测施加机械力来增强MSCs再生性能的能力。利用这个系统,我们进行了一系列强有力的筛选实验,检测了生化因子、药物抑制剂和生物力学力量之间的协同作用,将MSCs诱导成具有增强再生特性的血管细胞样表型。确定了诱导骨髓间充质干细胞中YAP和Smad2/3介导的信号通路最大激活的特定机械条件。利用高通量筛选结合应用机械负荷,确定了协同激活这些途径的药物抑制剂。对机械处理和药物联合处理的细胞表型的严格分析显示周细胞和内皮细胞标记物的表达增加。在皮下移植和后肢缺血的动物模型中,这些经过优化处理的MSCs在体外表现出增强的周细胞和内皮细胞行为、血管生成可溶性因子的产生以及增强血管生长的治疗潜力。

细胞在定制的培养板上培养,这些培养板安装在一个施加菌株的系统上。精确校准施加在每个孔上的机械应变。用慢病毒转染人骨髓间充质干细胞(第3代)表达荧光素酶基因。利用生化因子长期调节hMSCs。进行免疫细胞化学染色。用BD-LSR-II-Fortessa流式细胞仪(BD-Biosciences)检测群体荧光信号。使用血管生成因子抗体阵列分析培养基中可溶性因子的产生。为了评价条件化MSCs的体内反应,将细胞植入小鼠皮下。为了评估MSCs诱导的血管生成潜能和功能恢复,在小鼠中建立了后肢缺血小鼠模型。为了确定内皮细胞和血管平滑肌细胞的特征基因集,下载了多个RNA序列数据集。

通过机械生物学筛选,研究确定了增强MSC治疗活性和诱导向周细胞和ECs联合标记的新表型分化的条件。MSCs中周细胞的增加得到了多种正交技术的支持,包括多标记流式细胞术、体外功能测定、RNA-seq基因表达分析和动物模型。有几项研究支持血管周围细胞在体内可能表现为间充质干细胞。然而,其他研究表明周细胞不是干细胞。本研究表明,在培养的MSCs中可以存在一种混合的周细胞-内皮表型,并且可以通过机械负荷和抑制的联合作用来增加这种表型。

在机械生物学筛选实验中,发现了通过p-Smad2/3和YAP/TAZ通路增加信号传递的条件。在以前的许多研究中,这些通路与MSC分化为其他MSC表型有关。然而,这些通路在血管生成和周细胞功能中也很重要。在血管生成和血管发育过程中,通过YAP/TAZ的信号在毛细血管尖端细胞中增强。此外,YAP和TAZ的周细胞特异性敲除通过减少HGF54的产生破坏了它们对肺泡形成的协调,HGF54是一种可溶性因子。血管稳定需要通过Smad2/3发出信号,调节周细胞/内皮细胞相互作用,TGF-β信号增强周细胞/间充质细胞与ECs57的联系。高通量筛选的激活这些通路的化合物增强了MSCs的周细胞和EC样表型,而没有显著增加成骨或成脂肪分化。此外,这些处理增强了HGF的表达和MSCs产生的可溶性因子的血管生成潜能。研究表明YAP和Smad2信号在调节MSC表型和再生特性中起着复杂的作用。虽然我们的筛选分析侧重于这些途径的激活,但未来的筛选可以直接针对周细胞的增强。

使用流式细胞术分析多种标记物的结果表明,在广泛的治疗下,MSCs的纯EC分化很少。鉴于对周细胞-内皮细胞杂交表型的发现,即使存在多种内皮标记物也不排除MSCs也表达壁标记物的可能性。即使在最理想的条件下,在没有周细胞标志物的情况下,只有约8%的MSC表达内皮标志物。因此,研究表明,以前关于MSCs中内皮细胞分化的发现应该从需要进一步研究排除杂交表型的角度来看待。

研究结果表明,MSCs可以通过周细胞和ECs标记物的高表达而形成增强的表型。诱导需要使用复杂的机械应变波形和药物治疗。在没有施加机械力的情况下,无法从MSCs获得更好的再生性能。考虑到MSC行为的广泛变化以及由于疾病和衰老而导致的活性丧失,一种潜在的方法可能是首先从患者中分离MSC,并使用类似于本研究中所用的机械生物学筛选来优化所需的表型。
