NEJM:王洪生和王宝玺团队在预防麻风提出中国方案!

该研究结果为国际首次报道,证实了单剂量利福喷丁对于麻风家内接触者的具有预防保护作用,可以大幅度减少麻风家内接触者发病。

中国医学科学院皮肤病医院(中国医学科学院皮肤病研究所)王洪生和王宝玺课题组在麻风高危人群的化学预防研究中取得重要研究进展,相关研究成果“Single-Dose Rifapentine in Household Contacts of Patients with Leprosy”于2023年5月18日在国际著名医学期刊NEJM《The New England Journal of Medicine》(2021. IF: 176.082)在线发表。王洪生教授与王宝玺教授并列为通讯作者,王洪生教授与王乐博士并列论文第一作者。中国医学科学院皮肤病医院(中国医学科学院皮肤病研究所)为第一作者单位。

麻风病是由麻风杆菌引起的慢性传染病,可造成患者残疾和劳动力丧失,给患者家庭和社会带来沉重的负担,也引起社会恐惧和歧视等问题。自1982年全球推广麻风联合化疗(MDT)以来,麻风流行已得到明显控制。但近10余年来,全球每年仍有超过20万新发患者。而我国在2020年底,每年也仍有400余例新发麻风病患者,其中30%来自于家内接触者。国外报道应用单剂量利福平进行麻风接触者的化学预防并获得成功,但在国内尚未推广。因此,在我国目前麻风低流行状态下,结合我国麻风流行特点,探索出一种预防我国麻风家内接触者发病的方法,是实现消灭麻风这一宏伟目标而急需解决的问题。这种防治工作的迫切需求极大地推动了我国麻风病高危人群利福喷丁和利福平化学预防队列项目的研究。

该研究为群随机对照研究,通过在云南、贵州、四川和湖南四省建立大样本前瞻性研究队列,首次探讨了单剂量利福喷丁的保护作用。研究共纳入207个县(区),随机分为利福喷丁、利福平和对照组。研究对象是麻风家内接触者,研究的主要结果是比较各组家内接触者接受干预后4年内的麻风发病率。具体方法是基于意向性治疗(Intention to treat, ITT)人群,采用广义估计方程(Generalized estimating equation, GEE)进行回归分析,并以累计发病率比值(Cumulative incidence ratio, CIR)和调整后的95%CI进行报告。同时也做了符合方案数据分析(Per-Protocol population)和敏感性分析(Generalized negative binomial model和Zero-inflated negative binomial model)。

该研究的入组时间从2015年持续至2018年,最终入组7450人,2022年全部完成主要终点随访。其中2331例为利福喷丁组,2760例为利福平组,2359例为对照组。研究主要终点,ITT分析显示利福喷丁组4年的麻风发病率明显低于对照组(CIR, 0.16; multiplicity adjusted 95% CI, 0.03 to 0.87, P=0.02),而利福平和对照组差异不明显(CIR,0.59; multiplicity adjusted 95% CI, 0.22 to 1.57; P=0.23);Per-Protocol分析显示利福喷丁组4年的麻风发病率下降了92% (CIR, 0.08; 95% CI, 0.01 to 0.63),利福平组4年的麻风发病率下降了72%(CIR, 0.28; 95%CI, 0.10 to 0.76)。

安全性终点显示,利福喷丁组未出现明显不良反应。亚组分析显示,化学预防干预措施对于男性家内接触者、多菌型麻风患者的家内接触者、新发麻风患者的家内接触者保护作用更明显。

该研究结果为国际首次报道,证实了单剂量利福喷丁对于麻风家内接触者的具有预防保护作用,可以大幅度减少麻风家内接触者发病。该研究成果为麻风家内接触者暴露后化学预防提供了高质量的循证医学证据,为进一步丰富和完善我国以及全球其他麻风流行国家的麻风防治政策提供了重要参考,为我国及全球消灭麻风提供了新技术、新方案,为实现习近平总书记在致第19届国际麻风大会的贺信中提出的:“促进消灭麻风目标早日在中国实现,为全球消灭麻风作出贡献”夯实了基础。

中国医学科学院皮肤病医院麻风中心团队,以及云南、贵州、四川和湖南省的麻风防治人员为该研究作出了重大贡献。复旦大学儿童医院临床研究中心的严卫丽教授,北京大学公共卫生学院流行病学与卫生统计学系的李立明教授和余灿清教授也为该研究的设计、数据统计与分析提供了重要指导意见。该研究得到国家卫生计生委公益性行业科研专项(201502008)、医科院创新工程项目(2016-I2M-1-005, 2017-I2M-B&R-14)等科研项目的资助。

王洪生教授团队长期从事麻风相关的基础、临床、防治与转化研究。除了在麻风化学预防领域取得了创新性成果外,基础研究方面,发现了麻风氨苯砜超敏反应综合征(DDS-DIHS)与人白细胞抗原HLA-B*13:01分子高度相关(J Invest Dermatol. 2013. IF:7.590),并进一步发现一种新的p-i HLA/TCR药物超敏模型,阐明了氨苯砜药物超敏综合征发病的分子免疫机制(Journal of Biomedical Science. 2022. IF:12.771);临床诊疗方面,报道了多例特殊和罕见的麻风病例(BMJ.2022. IF:93.33)及新型分枝杆菌感染病例(EID.2021. IF:16.13);麻风防治方面,建立了中国麻风密切接触者人群麻风风险预测模型(EBioMedicine. 2021. IF:11.205)等;以前期为基础,获得多项分子免疫诊断新型发明专利并进行转化,与中国医学科学院系统医学研究所联合研发了麻风分枝杆菌核酸检测试剂盒、HLA-B*13:01基因检测试剂盒,成功获得欧盟CE认证。通过系统性工作,进一步推动麻风病的早期发现、精准诊治、早期预防与预警,为我国进一步丰富和完善麻风防治策略提供了依据,为在我国尽快全面消除麻风危害贡献了重要力量。
