动脉僵硬与血管衰老的早期阶段有关,是急性或慢性心血管疾病(CVD)发作的独立预测因子。近年来,由于他汀类药物具有降脂作用,可以改善血管系统,有助于降低动脉僵硬度,因此,他汀类药物已成为全球范围内初级预防心血管疾病处方最多的药物。同时,体育锻炼已被证明可有效改善动脉僵化 。虽然,他汀类药物和体育锻炼都已证明了对血管健康的有益作用,但体育锻炼在临床上的价值显然被低估了。

他汀类药物作为心血管疾病的一线用药,与很多因素相关,比如便捷的处方开出、基层医疗机构就诊时间长,锻炼的影响被低估,以及很少受过正确处方的培训等等。近日,发表在Plos Med的一项研究显示,他汀类药物和体育锻炼都可有效降低动脉僵硬,中度他汀类药物使用剂量和高强度运动似乎更为有效。



在这项研究中,研究人员系统地检索了MEDLINE、Embase、SPORTDiscus、Cochrane Central Register of Controlled Trials、Cochrane Database of Systematic Reviews和Web of Science等数据库,共纳入了来自全球的22项有关他汀类药物或体育锻炼对动脉僵化的影响的研究,其中,18项随机对照试验和4项非随机实验研究,包含1307名高心脏代谢风险患者。根据他汀剂量和ACC/AHA强度分类,他汀类药物的干预措施按类型和剂量分为高,中和低。体育锻炼干预是根据美国运动医学学院的处方运动强度分为高,中或轻度。

结果显示,他汀和体育锻炼研究的患者总数相似,分别为587名和720名。体育锻炼干预组的参与者的心脏代谢风险基线低于他汀类药物干预组。使用3种他汀类药物剂量和2种强度的体育锻炼对动脉僵硬度的影响,中度他汀类药物剂量和高强度运动似乎更为有效。他汀类药物的高剂量显示较高的SUCRA(74%)。中度他汀类药物剂量和高强度运动显示出第二和第三高的累积排序曲线下的面积(SUCRA)分别为67%和60%。



总之,通常情况下,适度的他汀类药物剂量和高强度运动均可改善动脉僵硬度。因此,高强度运动干预措施应被视为中度他汀类药物治疗的替代方案或可行的替代方案。事实上,越来越多的专家建议规定的“锻炼药片”作为预防策略,减少心血管疾病。
