目的:认知能力下降和骨质疏松症经常同时存在,一些证据表明两者之间存在因果关系。然而,没有数据表明认知能力下降、骨丢失和骨折风险之间的纵向关系,并且独立于衰老。本研究旨在确定:(1)认知能力下降和骨丢失之间的关系;(ii)在前5年的简易智力状态检查中,临床上显著的认知功能下降(≥3分),以及随后10年的骨折风险。

方法:从1997年到2013年,加拿大多中心骨质疏松研究对1741名年龄≥65岁的女性和620名男性进行了随访。使用混合效应模型估计认知能力下降和(i)骨丢失之间的关系;(ii)使用调整后的Cox模型估计骨折风险。

结果:95%以上的参与者在基线时认知正常(MMSE≥24)。男女MMSE的年百分比变化相似(女性为0.33,四分位区间[IQR]为0.70 - +0.00;男性为0.34,IQR: 0.99至0.01)。在多变量调整后,认知能力下降与女性骨质流失相关(6.5%;95%置信区间[CI],MMSE每下降一个百分点,从基线下降3.2%到9.9%),但不包括男性。大约13%的参与者在第5年经历了显著的认知能力下降。在女性中,骨折风险显著增加(多变量风险比[HR],1.61;95%可信区间,1.11 ~ 2.34)。纳入人员太少无法进一步分析。
