JAMA Surg：创伤登记纳入与排除老年人群预后差异研究

研究认为，现行的创伤登记制度存在较大缺陷，漏诊率较高，许多高风险老年患者未能得到及时的救治

创伤登记是创伤系统中评估和改善受伤患者护理的主要数据机制。近日研究人员就创伤登记纳入与排除人群的预后进行了考察。

研究人员考察了2011年间急诊医疗服务机构向创伤和非创伤中心转诊的65岁及以上患者数据，研究的主要终点为死亡时间和原因。

8161例患者中，5579例（68.4%）为女性，平均年龄为82.2岁。共有1720名老年人（21.1%）与创伤登记记录相匹配。各个创伤登记处未捕获的严重受伤患者率从18%（38名患者中的7名患者，腹部盆腔损伤评分为3或更高）至80.0%（2241例患者中有1792例肢体损伤评分为3分或更高），186例患者中有68例需要进行大型非骨科手术(36.6%)和在2325名需要进行骨科手术的病人中，有1809人(77.8%)没有被列入创伤登记。创伤登记处漏诊的重伤或大型手术患者中，36.4%在创伤中心接受治疗。当对登记组和非登记组进行12个月的随访后，非登记组院死亡中有93例（49.5%），总死亡1531例（81.1%）。

研究认为，现行的创伤登记制度存在较大缺陷，漏诊率较高，许多高风险老年患者未能得到及时的救治。
