Nat Med:科学家揭示肥胖与糖尿病之间的分子联系

肥胖能够引起炎症反应,从而导致2型糖尿病。但是对于验证是如何引起糖尿病,以及如何进行防治目前还没有确切的答案。美国UCSD医学院的研究人员发现炎性分子LTB4能够促进胰岛素抵抗-引发2型糖尿病的第一步。另外,研究人员发现通过遗传学手段敲出响应LTB4的受体,或者通过药物干预的方式阻断LTB4受体的功能会提高肥胖小鼠对胰岛素的敏感性。相关结果发表在2月23日nature medicine 杂志上

"这项研究十分重要,因为它揭示了2型糖尿病发生的根源"。本篇文章的通讯作者Jerrold M. Olefsky博士说道,"现在我们了解了LTB4是引发胰岛素耐受的的炎症因子,我们可以因此通过抑制LTB4的活性打破肥胖与糖尿病之间的联结。"
 
根据Olefsky博士的研究,过度肥胖,尤其是在肝脏中脂肪的过度积累会激活局附近的巨噬细胞。这些激活后的巨噬细胞进一步释放LTB4以及其它炎性分子从而吸引巨噬细胞的浸入。这一正向反馈调节机制会产生更多的LTB4以及激活更多的巨噬细胞。
 
炎症反应在机体抵抗外源感染时起到了正面的作用,然而当慢性炎症反应时,例如肥胖,过多的LTB4会激活巨噬细胞以外的其它细胞,像肝细胞,脂肪细胞,肌肉细胞等。这一情形下,这些细胞受到刺激从而对胰岛素产生了耐受。
 
当Olefsky与他的研究团队在小鼠模型上成功阐释了这一机制后,他们计划寻找能够抑制LTB4的方式。首先,他们他们利用遗传学手段建立了缺失LTB4受体的小鼠,结果显示这一小鼠在肥胖状态下代谢水平大幅提高。另外,他们还利用一种小分子抑制剂用于抑制LTB4受体活性,结果显示这一小分子机制效果与遗传突变的效果相当。,
 
"当我们通过遗传学手段或药物阻断的方式破坏LTB4引起的炎症循环后,我们得到了非常好的症状改善。虽然这些小鼠依然肥胖,但是他们的状态明显好了许多", Olefsky博士说到。
