高迁移族蛋白B1在创伤中的研究进展

高迁移族蛋白(HMG)最早由Johns于20世纪70年代在小牛胸腺中发现,因其相对分子量小(25000-30000),在PAGE电泳迁移速度快而得名,而HMGB-1是其主要家族成员之一。近年来,HMGB-1因在病理状态下具有促炎作用而备受关注。目前,HMGB-1已被报道广泛参与脓毒血症、关节炎、胰腺炎、自身免疫性疾病、肿瘤、器官纤维化等多种疾病的发生和发展,然其在创伤中的作用却未被系统阐述。现

高迁移族蛋白(HMG)最早由Johns于20世纪70年代在小牛胸腺中发现,因其相对分子量小(25000-30000),在PAGE电泳迁移速度快而得名,而HMGB-1是其主要家族成员之一。近年来,HMGB-1因在病理状态下具有促炎作用而备受关注。目前,HMGB-1已被报道广泛参与脓毒血症、关节炎、胰腺炎、自身免疫性疾病、肿瘤、器官纤维化等多种疾病的发生和发展,然其在创伤中的作用却未被系统阐述。现就HMGB-1在创伤中的研究进展做一综述。

HMGB-1简介

HMGB-1是一种广泛存在于真核细胞核内,分子量为30kD,结合于DNA的非组蛋白。啮齿类动物与人的HMGB-1由215氨基酸组成,其结构高度保守,其功能是帮助稳定核仁的结构和调节基因的表达。HMGB-1包含2个与DNA结合的结构域,既A区与B区。其中,B区是发挥功能的关键结构。

正常生理条件下,HMGB-1组成性表达于有核细胞内。当细胞损伤或坏死后,HMGB-1被动释放到细胞外。巨噬细胞、单核细胞、中性粒细胞、小胶质细胞等受内毒素、IL-1、TNF-α等炎性介质刺激后,可使位于核内的HMGB-1转运至胞质,进而通过甲基化、磷酸化和乙酰化修饰后主动分泌至细胞外。细胞外的HMGB-1作为一种“危险相关性分子模式”(DAMPs)或“警报素”(alarmins),具有较强的促炎作用。它能与靶细胞上的受体TLR2、TLR4、TLR9和RAGE结合,并通过MyD88依赖性和非MyD88依赖性信号通路,“瀑布样”激活其下游一系列蛋白激酶和转录因子,引起ROS(reactiveoxygenspecies)释放增多、炎症因子和其他免疫调节分子基因的表达,从而在多种疾病的发生发展中发挥负面的作用。

HMGB-1在创伤中的研究进展

近年来,较多国内外研究发现HMGB-1不同程度参与介导和加重颅脑创伤、脊髓损伤、烧伤、出血性休克、肺挫伤、骨折等创伤后的炎症反应。Peltz等观察发现,23例ISS评分≥15创伤患者循环中HMGB-1水平在1h时比健康志愿者高出30倍。与HMGB-1在脓毒血症中扮演“晚期炎症介质”不同,HMGB-1在创伤的早期就被释放并参与创伤后的炎症反应,介导继发性损伤。其在不同类型创伤中的研究情况如下。

外伤性脑损伤(TBI)

HMGB-1广泛表达于海马齿状回细胞系、嗅球和端脑脑室细胞系、成人的神经元、星形胶质细胞、少突细胞和脉络丛内皮细胞中。TBI后,HMGB-1由坏死和损伤的神经细胞释放。细胞外的HMGB-1激活免疫细胞释放多种炎症介质,一方面介导神经细胞凋亡;另一方面通过多种复杂的病理生理机制,对脑组织造成二次损伤。

HMGB-1对神经元的凋亡有介导作用且与RAGE-P38MAPK和RAGE-ERK信号通路密切相关。Kim等发现,HMGB-1在NMDA处理的原代培养基中被激活,新鲜的培养基与之混合将导致神经细胞的凋亡,而用HMGB-1SiRNA转染或抗HMGB-1抗体中和后的NMDA处理培养基与新鲜的培养基混合后神经细胞凋亡明显被抑制。随后有序的RAGE和HMGB-1联合免疫沉淀实验提示RAGE-P38MAPK和RAGE-ERK信号通路在HMGB-1介导的神经凋亡中有重要的作用。

HMGB-1不仅参与介导神经元凋亡,而且能通过复杂的机制加重脑水肿。Laird等研究发现HMGB-1/TLR4通路介导的小胶质细胞激活对创伤后脑水肿的发生发展有重要的贡献作用。TBI后大鼠CSF中HMGB-1水平显著升高,并与颅内压水平息息相关。升高的HMGB-1与小胶质细胞表面的TLR4受体结合,上调IL-6的表达。IL-6随后作用于星形细胞,促进其生成水通道蛋白-4(AQP4),加剧脑水肿。而基因层面或药理学层面的TLR4抑制可明显减轻神经元的炎症反应、局限创伤后颅内水肿。

Okuma等在体外血脑屏障系统模型中发现,重组HMGB-1直接影响血管上皮细胞和周皮细胞,导致血脑屏障的通透性升高,加重脑水肿。抗HMGB-1单克隆抗体随后也被发现能显著减轻T2加权MRI下大鼠脑损伤和脑水肿的表现,维持血脑屏障的完整性,抑制TNF-α、IL-1β等炎症介质的表达并改善大鼠TBI后的神经功能。

HMGB-1介导神经细胞凋亡和造成TBI后二次损伤的特性提示未来其对临床上TBI的诊断、治疗和预后判断有重要的参考价值。一项观察性研究发现,重型颅脑损伤组患者血液中HMGB-1显著升高,且1年内死亡组循环HMGB-1水平显著高于存活组。

此外,丙酮酸乙酯(EP)被报道可抑制HMGB-1、TLR4的表达和下调NF-κB与DNA的结合能力,显著改善TBI模型大鼠颅内水肿和神经细胞凋亡,然而其临床价值有待进一步探索。

急性脊髓损伤(SCI)

在急性SCI的病理生理过程中,坏死的神经组织释放多种促炎细胞因子,促进免疫细胞活化和迁移,加重局部损伤。HMGB-1是其中重要的炎性介质。在啮齿动物脊髓挤压伤和缺血损伤的模型中,HMGB-1水平在局部损伤的脊髓组织和外周循环中显著升高。局部升高的HMGB-1对脊髓神经细胞的凋亡有重要的介导作用,而外周循环中升高的HMGB-1可作用于循环中的免疫细胞和与靶器官上的相应受体结果,加重炎症反应,造成远隔器官损伤。急性SCI后,继发神经损伤的主要机制是神经细胞的凋亡。

神经细胞在被包括HMGB-1在内的一系列炎性损伤促进因子的作用下出现凋亡。一项研究发现HMGB-1、RAGE的表达在脊髓损伤后且神经细胞凋亡达到高峰前被上调。进一步研究提示HMGB-1至少部分参与介导脊髓神经元凋亡并与HMGB-1/RAGE依赖性的NF-κB-MAPK信号通路密切相关。虽然相关研究取得进展,但HMGB-1介导神经细胞凋亡更详细的分子生物学机制有待进一步被阐述。一些研究者注意到细胞内的HMGB-1可能与运动神经元轴突再生和血管再生密切相关。
Fang等在成年斑马鱼完全性脊髓横断损伤再生模型中发现,HMGB-1在损伤的运动神经元及血管内皮细胞中表达增加,而反义HMGB-1核苷酸处理可显著减少34%的运动恢复和34%轴突再生。脊髓缺血损伤模型小鼠中抑制HMGB-1可减少脊髓梗死周围微血管密度和抑制运动神经元的修复能力。斑马鱼胚胎细胞内HMGB-1基因敲除可显著减少前脑表达酪氨酸氢化酶神经元的数量和减少脑组织体积。体外实验也发现,HMGB-1能促进N18神经母细胞瘤细胞和皮层神经元轴突的生长与小脑颗粒细胞的再生。这些结果表明HMGB-1在中枢神经系统再生和修复方面有着重要的作用。如何减少HMGB-1在急性SCI中的负面作用是研究的另一热点。

Kikuchi等认为HMGB-1是改善脊髓损伤后神经功能的新颖治疗靶点。已有研究证实N-乙酰-5-甲氧基色胺、丙酮酸乙酯、氢气可以抑制HMGB-1在啮齿动物损伤脊髓中的释放,减少运动神经元的凋亡和提高运动功能。最近一项研究显示,高压氧治疗(HBO)能在脊髓损伤后7~14d显著降低HMGB-1mRNA及蛋白的表达。HBO可以抑制HMGB-1的表达,一定程度上阻断RAGE、TLRs依赖性的NF-κB活化,改善脊髓损伤动物远期的神经功能。

烧伤

近年来一些研究显示重度烧伤后循环中HMGB-1水平与脓毒血症、MODS的发生发展及患者的预后密切相关。重度烧伤后HMGB-1的释放一方面会因其促炎的特性而增加患者并发脓毒血症、MODS的风险,另一方面HMGB-1也是免疫抑制细胞因子。升高的HMGB-1可抑制Th-1细胞和巨噬细胞释放促炎细胞因子,导致T细胞免疫功能障碍。Dong等通过分析35例烧伤体表总面积>30%患者发现,大面积烧伤患者伤后血浆HMGB-1含量与细胞免疫功能指标包括T淋巴细胞增殖活性、IL-2含量、CD4+/CD8+T细胞比值呈显著负相关关系。严重烧伤患者血浆HMGB-l水平的持续升高对机体细胞免疫功能异常具有显著影响。

失血性休克(HS)

HS是严重创伤患者常见的并发症和直接的死亡原因之一。HS及其复苏过程中再灌注损伤(HS/R)往往能导致MODS的发生。近年来HMGB-1在HS/R损伤中的作用越来越受关注。

随后诸多国内外研究发现,HMGB-1在HS/R诱导的急性肺损伤(ALI)中扮演重要的角色。Kim等报道大鼠肺组织内HMGB-1水平在HS/R后4h显著升高,并一直持续到72h。抗HMGB-1抗体处理后,肺组织细胞核内NF-κB的移位被明显抑制,包括IL-6、IL-1β在内的促炎细胞因子的水平显著降低,中性粒细胞募集减少,肺渗透压降低,肺功能得以改善。由于环磷酰胺诱导的中性粒细胞缺乏的大鼠HS后肺组织内HMGB-1水平无明显变化,中性粒细胞被认为是HS后HMGB-1的主要来源。

Fan等进一步研究后认为HS/R后,HMGB-1/TLR4信号通路依赖性的NADPH氧化酶活化及ROS释放是中性粒细胞介导ALI的重要机制。他们发现,野生型小鼠中,HMGB-1通过磷酸化p47phox单元来激活中性粒细胞中的NADPH氧化酶,促进ROS的释放。TLR4缺失的C3H/HeJ小鼠,NADPH氧化酶活化显著减少,ROS释放显著降低。抗HMGB-1抗体可显著抑制中性粒细胞p47phox单元磷酸化。体外实验也提示,重组HMGB-1可激活中性粒细胞并导致MyD88-IRAK4-p38MAPK和MyD88-IRAK4-Akt信号通路介导的ROS释放增加。

HMGB-1还被发现与HS/R后肠屏障功能障碍的发生和发展密切相关。Yang等在大鼠HS模型中发现,大鼠血液中HMGB-1水平显著升高,回肠粘膜通透性增加,肠内细菌向肠系膜淋巴结移位的风险升高。而这些改变均能被抗HMGB-1中和性抗体抑制。临床观察性研究也发现与9名健康志愿者对比,25例HS/R后肠粘膜功能障碍患者外周循环中HMGB-1水平显著升高。此外,HMGB-1也被报道可以增强人结直肠腺癌细胞肠上皮样上皮细胞层的通透性。

肺挫伤

胸部创伤常常导致肺挫伤和脓毒血症。Bitto及其同伴对24例X线片和CT确诊肺挫伤患者进行比较后发现,肺挫伤患者血液和支气管肺泡灌洗液中HMGB-1mRNA的浓度显著高于对照组,并与ISS、RTS评分和病人的转归有正相关关系。严重肺挫伤患者支气管肺泡灌洗液巨噬细胞中HMGB-1表达显著上升,与肺挫伤患者的严重程度密切相相关。但HMGB-1在肺挫伤发生发展过程中的具体角色与机制有待进一步被阐明。

股骨骨折

一项小鼠双侧股骨骨折的实验研究显示,血液中HMGB-1水平在骨折后1h内升高。在野生型小鼠抗HMGB-1中和性抗体处理后,血液中与创伤严重程度相关的IL-6、IL-10水平和ALT水平显著降低,肝脏和肠粘膜细胞中NF-κB与DNA的结合能力被显著抑制。而TLR4突变型的小鼠骨折后系统性炎症反应和肝损害均较轻,且抗HMGB-1中和性抗体未能明显抑制炎症反应和肝损害。该研究暗示HMGB-1/TLR4信号通路在加重骨折后炎症反应和介导远隔器官损伤中有重要的作用。

小结

HMGB-1作为重要的炎症介质,广泛参与了各种类型创伤的病理生理过程,加重创伤后的炎症反应,造成二次损伤。HMGB-1在TBI、急性SCI、烧伤、HS/R等创伤中的研究已取得相应进展,但其具体的信号机制及与其他炎性介质的相互作用原理则有待进一步被研究。临床上HMGB-1是否可用于判断TBI、烧伤、HS/R及肺挫伤等患者的病情和预后尚无统一认识。已有一系列抑制创伤后HMGB-1释放,从而改善创伤动物结局和转归的实验研究,但其治疗效果还有待进一步探索。鉴于HMGB-1参与各型创伤的病理生理过程,探讨其在各型创伤中扮演的具体角色具有积极的意义,并将有助于提高创伤的救治水平。
