这是首次全面研究产前接触某些化学物质和化学混合物与非酒精性脂肪肝之间的关系。研究人员使用细胞角蛋白-18作为儿童这种疾病的新标记。这些发现最近发表在JAMA Network Open上,强调了理解产前暴露于环境化学品是非酒精性脂肪肝的风险因素的重要性,这一问题在儿童中迅速变得越来越普遍,并可能导致成年后严重的慢性肝病和肝癌。

非酒精性脂肪肝是世界上最普遍的肝脏疾病之一,越来越多的儿童被诊断为非酒精性脂肪肝,影响了6%到10%的儿科人口和大约34%的肥胖儿童。许多农药、塑料、阻燃剂、有害金属和其他环境污染物都属于干扰内分泌的化学品。例如全氟烷基物质(PFAS),也被称为“永久化学品”,用于不粘锅和食品包装,以及多溴二苯醚(PBDEs),用于家具和婴儿产品的阻燃剂。干扰内分泌的化学物质会干扰人体的激素和代谢系统。多项实验研究表明,接触这些化学物质可导致肝损伤和非酒精性脂肪肝;然而,到目前为止,产前混合接触这些化学物质的潜在影响还没有在人类中进行研究。

在这项研究中,研究人员从2003年到2010年测量了1108名孕妇血液或尿液中的45种化学物质。这些化学品包括内分泌干扰化学品,如PFAS、有机氯和有机磷农药、增塑剂(苯酚、邻苯二甲酸酯)、多溴二苯醚和对羟基苯甲酸酯。当这些孩子长到6岁至11岁时,科学家测量了这些孩子血液中酶和细胞角蛋白-18的水平,这些水平表明孩子患肝病的风险,发现在怀孕期间更多地暴露在环境化学物质中的孩子,这些生物标志物的水平更高。
