在一般人群中,老年人与较短的白细胞端粒长度和感染高风险相关。在最近的异体造血干细胞移植治疗严重再生障碍性贫血的研究中,供体较长白细胞端粒长度与受者改善的生存相关。这些结果提示白细胞端粒长度可能是机体免疫能力的标志。近日,血液病领域权威杂志Haematologica上发表了一篇研究文章,研究人员验证了一个假说,即较短的白细胞端粒长度与感染性疾病住院和感染相关死亡较高风险相关。

研究人员采用定量聚合酶链反应测定了75309例来自于一般人群个体的外周血白细胞端粒长度,随访时间为23年。在随访期间,9228名受试者因感染而住院,1508名受试者发生感染相关的死亡。对常规感染性疾病危险因素进行调整后,研究人员发现较短的端粒长度与任何感染(白细胞端粒长度每缩短一个标准差的风险比为1.05;95%可信区间为1.03-1.07)和肺炎(1.07;1.03-1.10)高风险相关。感染相关的死亡对应的任何感染风险比为1.10(1.04-1.16)以及肺炎的风险比为1.11(1.04-1.19)。端粒长度与皮肤感染、尿路感染、脓毒症、腹泻病、心内膜炎、脑膜炎或其他感染的风险无关。

总之,该研究结果表明,白细胞端粒长度可能是免疫能力的一个标志。仍需要进一步的研究以确定通过捐助者的白细胞端粒长度选择捐助者时是否会减少异体造血干细胞移植受者感染的风险。
