IJLH:血小板整体制剂的电子显微镜检查对血小板致密颗粒数定量-对致密颗粒缺乏引起的疑似血小板功能障碍的诊断意义

致密颗粒(DG)缺乏症(DGD)是一些血小板功能障碍(PFD)的特征,其患病率与冯维勒布兰德病相似。大多数实验室使用整体血小板制剂和电子显微镜(EM)评估DGD。本研究评估了这项检测的经验以及DGD和出血之间的关联。

研究人员检查2006-2017用于患者的致密颗粒EM记录并同时测试对照,并且对于重叠的PFD研究群组来评估发现及其与出血的关系。

结果发现与对照样本相比,更多患者的DG计数减少(6.5vs 0.3%,P <.01)。DG计数显示与年龄或平均血小板体积无关,并且具有可接受的受试者内变异性,DGD高于对照参与者(28%对12%)。重复试验确认所有DGD患者的初始DG计数<4.0 /血小板,那些严重程度较轻(4.0-4.8 DG /血小板)或正常DG计数(≥4.9DG/血小板)的患者例外。对DGD聚集测定法和三磷酸腺苷释放测试分别仅有约52%和70%的灵敏度。EM确诊的DGD与较高的出血评分和出血性疾病相关。

结果表明,整体EM可用于评估由DGD引起的疑似PFD,并可检测与出血相关的异常。。
