J Dermatolog Treat：JAK抑制剂联合紫外光疗可更有效的治疗白癜风

白癜风是一种自身免疫性疾病，其特征是黑色素细胞的逐渐丧失，导致皮肤脱色。白癜风对患者有重大的社会心理影响，而且治疗方案有限，管理起来很有挑战性。

白癜风是一种自身免疫性疾病，其特征是黑色素细胞的逐渐丧失，导致皮肤脱色。白癜风对患者有重大的社会心理影响，而且治疗方案有限，管理起来很有挑战性。最近的研究表明，包括tofacitinib和ruxolitinib在内的JAK1/3抑制剂有很好的效果。但是迄今为止，大部分的文献都是基于小量的数据，缺乏明确的证据或指南。为了解决这些目前的知识空白，发表于J Dermatolog Treat的一项系统回顾和荟萃分析探究了白癜风对JAK抑制剂治疗的预期反应以及影响反应率的因素。

良好的反应被定义为再色素沉着>50%或由作者描述的"良好"或"优秀"的结果。部分反应被定义为一些再色素沉着<50%。

 

结果，研究共纳入9项符合条件的研究，涉及45个病例的个体患者数据。分析结果显示，57.8%的病例获得良好反应，22.2%的病例获得部分反应，20%的病例无反应或反应极小。如果按照部位进行分组，面部白癜风的良好反应率最高（70%），相比之下，四肢（27.3%）和躯干/非阳光暴露部位（13.6%）。同时进行的光疗与较高的总体良好反应率（p < .001）和面部良好反应率（p < .001）有显著关系。

综上所述，该研究结果表明，关于JAK抑制剂对白癜风的疗效的证据质量低，但是研究结果多为肯定。同时联合进行UVB光疗似乎可以提高JAK抑制剂对白癜风的疗效。未来需要进行大规模的随机研究来证实这些研究发现。
