尿蛋白质的假阴性和假阳性,你真的会鉴别吗?

医学检验指标日益增多,各专业组精细化管理,检验项目也变得相对独立,各专业的检验人员对其他亚学科检验指标的检测原理、临床意义缺乏全面了解,发出自相矛盾的结果会给临床带来疑惑。

尿蛋白质是尿液常规检测重要的检验项目,干化学检测的尿蛋白质是蛋白尿检测的最快捷、最简便方法,广泛运用于临床。随着科学技术的发展,尿液蛋白质的检测已几乎走向自动化。但工作实践证明自动化仪器的检测可造成一定的假阴性和假阳性,常会遇到尿蛋白定性和定量结果相矛盾,需要我们检验人加以辨别,和临床做好沟通解释。

尿蛋白质假阴性

案例经过:

患者凌某,男,62岁,因“反复关节疼痛10年余,加重5天”入院,患者自诉于10年前开始无明显诱因下出现反复踝关节及跖趾关节疼痛,发病时关节疼痛呈持续性胀痛,伴有关节皮肤潮红、肿胀、活动受限,自行药物治疗后可缓解,但症状反复,每年均有发作超过2次。现为进一步诊治到我院就诊,门诊拟"痛风性关节炎"收入肾内科。入院后尿蛋白质阴性,而24小时尿蛋白定量却惊人的达到1016mg/24h,这样矛盾的结果必定会给临床带来困惑。

案例分析:

尿常规检查尿蛋白质定性检测的主要是白蛋白(Alb)[1],而尿24小时定量检测的是尿中的总蛋白质,尿中蛋白质包括白蛋白、球蛋白、血红蛋白、本周蛋白和粘蛋白等,尿常规检查采用的干化学试带法蛋白反应区主要对尿中白蛋白灵敏,由于尿常规蛋白质对白蛋白敏感,对球蛋白的敏感仅为白蛋白的1/50~1/100,因此当尿液中升高的蛋白质非白蛋白时,尿常规检验尿蛋白质结果可能阴性,建议临床加做尿白蛋白肌酐比(ACR)也是正常的,而尿β2微球蛋白(β2-MG)是升高的。证明患者尿白蛋白是正常的,非白蛋白异常升高,患者尿液中蛋白质可能为溢出性蛋白。这种蛋白多半是来自肾小球滤过的分子量低于Alb的蛋白质如凝溶蛋白(本周蛋白)[2],在遇到球蛋白增多的病例如巨球蛋白血症,多发性骨髓瘤患者时,检测结果明显偏低,因此尿白蛋白不一定阳性。

案例二

尿蛋白质假阳性

案例经过:

门诊患者,女,12岁,曾因过敏性紫癜入院治疗,后继发紫癜性肾炎,出院后按医嘱门诊复查尿常规和24小时尿蛋白定量。患儿尿常规蛋白质4+,而24小时尿蛋白定量却是正常的。

案例分析:

过敏性紫癜(HSP)是以急性毛细血管炎为主的变态反应性疾病,常伴有不同程度的肾功能损害,尿液蛋白质检测有助于评价患者肾功能,对肾损害的早期诊断、病情监测和预后判断具有重要意义。患者尿硝酸盐和PH明显异常,与患者联系得知,患者的尿常规是从24小时尿液留取的,告知患者要重新留尿检查,但患者已回家,次日患者复查尿常规尿蛋白质为阴性。患者尿常规和24小时尿蛋白结果矛盾原因为尿液放置时间太长,大多数细菌可分解尿素释放氨,尿液呈碱性,而尿常规的尿蛋白质是干化学法,PH过高容易出现假阳性。有实验研究证明[3],当尿液PH>8.0,仪器干化学检测尿蛋白质容易出现假阳性。我们通过调节尿也酸碱性检测也证实,尿液PH升高致其尿蛋白质假阳性。

医院检验科的快速发展,医学检验指标日益增多,各专业组精细化管理,检验项目也变得相对独立,各专业的检验人员对其他亚学科检验指标的检测原理、临床意义缺乏全面了解,发出自相矛盾的结果会给临床带来疑惑,同时也有可能增加患者复检的经济负担,我们检验人应加强科室内部各专业组间的信息交流,增进对临床的了解和沟通,加强交流合作,促进临床对检验科的了解与信任,与临床交流沟通的过程中不断丰富自己的知识结构,提升工作质量与效率,减少临床医生的对检验结果的误解,为患者提供优质服务,同时得到他们的尊重[4]
