阿尔茨海默病(AD)的特点是存在淀粉样蛋白-β(Aβ)和tau病变,这些病变被认为在临床前阶段积累多年,导致在临床阶段观察到的神经变性和认知能力下降。Presenilin-1(PSEN1;OMIM 104311)基因的突变使个体在成年早期易患常染色体显性阿尔茨海默病(ADAD),显示出与晚发或散发性AD类似的病理和神经退行性变化。对ADAD大家族的研究,包括约6000名在世成员和约1200名PSEN1 E280A(Glu280Ala)突变携带者的哥伦比亚家族,使得临床症状发作前几十年与AD相关的生物和行为变化的特征得以确定。对这些家庭正在进行的研究继续为自然史研究和AD的预防治疗试验提供信息。


正电子发射断层扫描(PET)已经实现了Aβ和tau积累的体内特征和连续追踪。对散发性AD的Tau PET研究结果与死后研究暗示的Tauopathy的时空进展一致;即皮质Tau积累开始于内侧颞叶(MTL)分配皮层,随后与Aβ一起扩散到颞部和颞外新皮层(也称为 "等皮层")。

在ADAD中,tau的积累是否遵循类似的时空模式仍未解决。我们以前曾报道过,相对于来自哥伦比亚的非携带者,PSEN1 E280A携带者早在预期的轻度认知障碍(MCI)发病前6年就出现了MTL(内嗅皮层,EC)tau PET的异常升高,而且ADAD中tau PET的总体空间模式与散发性AD中的相似。另一项ADAD研究发现,tau PET仅在受损个体中升高,并提示与散发性AD相比,包括EC在内的MTL tauopathy可能较少参与ADAD


纵向PET研究已经证明了脆弱的散发性AD人群的病理变化,但在ADAD中,tau和Aβ PET的纵向测量以及大脑结构和认知功能的测量还没有被报道。

哈佛大学的Justin S. Sanchez等人,追踪了年轻的PSEN1 E280A突变携带者和年龄匹配的非携带者在2-4年内tau和Aβ的PET测量、大脑结构的磁共振成像(MRI)测量和认知评估的变化,这些都是来自哥伦比亚ADAD种群。

来自哥伦比亚的14名ADAD突变携带者(Presenilin-1 E280A)和15名年龄匹配的非携带者在2-4年的随访期间接受了2-3次Aβ(11C-匹兹堡化合物B)和tau(18F-flortaucipir)PET、结构磁共振成像和神经心理学评估。在携带者和非携带者之间比较成像和认知变量的年化变化率,并评估携带者内部基线测量和变化率之间的关系。

他们发现:纵向测量结果与ADAD相关变化的顺序一致,从Aβ积累开始(预期症状发生前16年,EYO),随后是脑内皮层(EC)tau(9 EYO)、新皮层tau(6 EYO)、海马萎缩(6 EYO)和认知能力下降(4 EYO)。

携带者中tau的积累率在顶叶新皮层中最为迅速(约9%/年)。基线的EC tau PET信号是预测携带者随后的新皮层tau累积和认知能力下降的重要因素。


该研究与横断面研究所暗示的ADAD的生物变化顺序一致,并强调EC tau作为早期生物标志物的重要性,以及ADAD中Aβ负担和新皮质tau积累之间的潜在联系。
