BMJ:循环维生素D浓度与7种主要肿瘤风险关系研究

研究认为循环维生素D浓度与各种类型癌症的风险之间存不存在线性因果关系,研究不支持维生素D缺乏症筛查和维生素D补充作为预防原发性癌症的策略

额外的维生素D补充对肿瘤的预防效果目前尚存在较大争议。近日研究人员考察了循环维生素D浓度与癌症风险相关的因果关系。

研究总计招募了70563例癌症患者,其中前列腺癌患者22898人、乳腺癌患者15748人,肺癌患者12537人,结直肠癌患者11488人、卵巢癌患者4369人,胰腺癌1896人、神经母细胞瘤患者1627。研究同时招募了84418名对照志愿者。研究考察4种与VD相关的单核苷酸多态性(rs2282679, rs10741657, rs12785878 和rs6013897)以确定循环25-羟维生素D的多个多态性评分。研究的主要终点是直肠癌、乳腺癌、前列腺癌、卵巢癌、肺癌、胰腺癌和神经母细胞瘤的风险。

研究发现,循环中的25-羟维生素D水平与7种肿瘤以及其亚型之间的关系不显著。25(OH)D浓度上升25 nmol/L,其结肠癌风险下降的比值比为0.92(95%,CI,0.76 -1.10),乳腺癌为1.05(0.89-1.24),前列腺癌为0.89 (0.77-1.02),肺癌为1.03 (0.87-1.23)。对于大部分肿瘤而言,VD浓度下降25 nmol/L,肿瘤发生的风险为1.20-1.50。
