JASN:颗粒物空气污染与CKD发病和ESRD进展风险之间有何关系?

暴露于PM2.5与CKD、eGFR下降和ESRD的发生风险之间有着显著的相关性。

空气中直径小于2.5µm的细颗粒物(PM2.5)水平升高与心血管事件和死亡风险增加相关,但其与慢性肾脏病(CKD)和终末期肾病(ESRD)风险之间的相关性仍是未知的。

近日,肾脏病领域权威杂志Journal of the American Society of Nephrology上发表一篇研究文章,研究人员与美国环保署和美国退伍军人事务部的数据库取得联系组建了包含2482737名美国退伍军人的观察性队列,并使用生存模型来评估PM2.5浓度与eGFR<60ml/min/1.73m2、CKD、eGFR下降≥30%和ESRD之间的相关性。

在中位随访8.52年期间。基础的县级PM2.5暴露以2004年的年均浓度为准,并随着时间变化每年进行更新数据。在基础暴露分析中(中位数为11.8[10.1-13.7]µg/m3),PM2.5浓度增加10µg/m3,与eGFR<60ml/min/1.73m3(风险比[HR]为1.21;95%可信区间[95%CI]为1.14-1.29)、CKD(HR为1.27;95%CI为1.17-1.38)、eGFR下降≥30%(HR为1.28;95%CI为1.18-1.39)和ESRD(HR为1.26;95%CI为1.17-1.35)的风险增加相关。在随时间变化的分析中,PM2.5浓度增加10µg/m3其与eGFR<60ml/min/1.73m2、CKD、eGFR下降≥30%和ESRD的风险增加同样相关。样条分析显示PM2.5浓度与肾脏疾病结局的风险呈线性关系。来自于美国国家航空航天局卫星数据的暴露估计得到的结果一致。

该研究的结果表明,暴露于PM2.5与CKD、eGFR下降和ESRD的发生风险之间有着显著的相关性。

Circulation:PM2.5可致人体应激激素升高

复旦大学公共卫生学院阚海东教授课题组带领硕士研究生李慧楚、副研究员蔡婧,在大气细颗粒物(PM2.5)健康危害机制研究中取得新进展。他们发现,PM2.5暴露可引起人体应激激素水平显著上升,并促进机体的脂类氧化以及糖类和氨基酸的代谢,进而产生血压升高、炎症反应等现象,危害心血管健康。该发现为防治PM2.5相关的健康风险提供了新思路。相关成果近日发表于国际著名期刊《循环》杂志上。
