15年来中国人群高胆固醇患病率增加1.9倍,达到14.4%

作者在讨论中指出,我国人群胆固醇升高是导致冠心病死亡率增加的重要原因,胆固醇水平的增高预示着未来心血管病疾病负担的增加,“应加强全民健康生活方式宣教,尤其是在中青年和广大的农村地区。遏制并进一步逆转全人群胆固醇水平的上升趋势,是减少全民心血管疾病负担的重要策略。”

研究发现,在1992-2007年间,平均总胆固醇水平男性从4.65 mmol/L增至4.96 mmol/L,女性从4.40 mmol/L增至5.35 mmol/L。随时间推移,男性和女性总胆固醇水平分布曲线均向右侧更高水平移动(图1)。

而著名的弗莱明翰心脏研究发现,随时间的推移,基线总胆固醇水平低者呈上升变化,而基线总胆固醇水平高者呈降低变化,该现象被称为“向均值回归”。看来,我国居民没有发生这一现象。

从性别角度看,女性35~44岁年龄段人群平均总胆固醇水平增加值最大,为0.95 mmol/L。从不同地区的角度看,总胆固醇平均值最低的农村人群平均总胆固醇水平增加值最大,为0.88 mmol/L。

作者认为,基线胆固醇正常者对患病人数的增加贡献大,“进一步证实联合采用高危策略和全人群策略,对有效降低国人高胆固醇血症患病率的重要性和必要性。”

研究者于1992年在多省市35~64岁人群中进行基线危险因素调查,并建立心血管病研究队列;2007年在该队列人群中再次组织危险因素调查。2次调查资料均完整的调查对象共5740名,对其血清总胆固醇进行分析。高胆固醇血症组定义为≥6.22 mmol/L。
