JACC:袖带式血压测量,结果可能并不真实?

最新发表在《JACC》上的一项研究显示:袖带式血压测量结果和动脉内血压有偏差,这对正确的高血压分类有不利影响。

澳大利亚Tasmania大学James Sharman博士说,“研究人员长期以来一直对袖带式血压测量的准确性表示怀疑,但缺少相关证据。据我们所知,这是对该问题严重程度的第一次综合性分析。”

Sharman及其同事共进行了3项患者个体水平的Meta分析,纳入从1950年~2016年间关于测量主动脉内BP,肱动脉内BP和袖带BP的研究。分析共包括74项研究,3073例参与者。

第一项Meta分析评估了主动脉和肱动脉之间的动脉内BP水平是否一致(Meta分析1)。结果发现肱动脉内SBP比主动脉高8.0 mmHg(P<0.0001),肱动脉内DBP比主动脉低1.0 mmHg (P=0.038)。

第二项和第三项Meta分析,分别观察袖带式血压测量是否准确反映肱动脉内BP(Meta分析2)或主动脉内BP(Meta分析3)。结果显示袖带测量得到的肱动脉内SBP和脉压差结果显着偏低(两者均P<0.0001),SBP的平均绝对差异为7.9 mmHg。此外,袖带测量的肱动脉内DBP结果显着偏高(P<0.0001)。

肱动脉袖带BP与主动脉内SBP之间仅发现较小的平均差异,研究人员指出,这可能是因为肱动脉内SBP高于主动脉,而袖带测量的结果又偏低,平衡了之间的差异。

不准确的后果

根据既往研究,BP测量出现5 mmHg的不准确,就可能导致美国每年约5000万人的BP错误分级。Sharman表示,一方面低估血压将影响心血管危险的正确评估,患者可能错过通过干预血压减少个人未来心血管事件的机会;另一方面,高估了血压可能导致过度的降压治疗。

袖带式测量结果的不确定对BP分级有不利影响,特别是会导致高血压前期(pre-HTN)和1期高血压(HTN)的血压范围高度偏差。根据美国成人高血压治疗指南(JNC7) BP分级,使用袖带血压 (正常,pre-HTN,1期HTN和2期HTN)分别仅60%、50%、53%和80%与肱动脉内血压一致。而使用主动脉内血压,其一致性分别为79%、57%、52%和76%。

更好的解决办法还在探索中

“这些数据表明袖带式测量可能不是评估高血压的最佳方法,非侵入方法评估中心血压可能会改善高血压的评估和管理,从而改善心血管预防。”欧洲高血压学会(ESH)血压监测和心血管变异性工作组主席、希腊雅典大学George Stergiou博士说。

虽然使用动脉导管能获得最精确的血压测量结果,但Stergiou表示这种方式太过于侵入性并不适用于日常临床实践。

Sharman说,“最新技术的发展提供了通过袖带非侵入性方式评估中心BP的设备,并且还有无袖设备也正在开发中。在预测与高血压相关的风险因素方面,这些测量方法可能比传统的BP监测更准确,但它们仍需要适当的临床验证。”

“与此同时,最好的选择是24小时动态检测和家庭监测以及自动化临床BP测量。这些方法虽然使用相同的袖带方法,但随着时间推移记录了较多血压结果,这可能会降低误差。事实上,已经有证据表明,与诊断中使用袖带测量的BP结果相比,通过上述方法能够更好地了解真实BP水平。”

Stergiou等人共同撰写的述评指出:“从科学和生理的角度来看,侵入性动脉内BP测量结果是有最意义的。但是,已经应用于世界上数十亿人,应用时间长达半个世纪的临床医学和高血压科学(BP作为生命体征或高血压病的标志物),已经不可避免地要以无创袖带式血压测量为基础。”

尽管BP测量存在这些不确定性,但“临床实践中的高血压治疗在医学上仍是一个伟大的成功”。更好的测量方法及其是否可以更好地改善心血管疾病的预防,这是今天科学界应该解决的“21世纪难题”。
