IBD: 手术会持续降低炎症性肠病患者肠道中的微生物组和代谢组的多样性

在微生物的作用和代谢组学在炎症性肠病(IBD)的发生发展中扮演重要作用已经得到确认,许多研究都在探究微生物在溃疡性结肠炎(UC)和克罗恩病(CD)的中特异性的标志物。

      在微生物的作用和代谢组学在炎症性肠病(IBD)的发生发展中扮演重要作用已经得到确认,许多研究都在探究微生物在溃疡性结肠炎(UC)和克罗恩病(CD)的中特异性的标志物。但很少有研究专门针对手术或其对代谢组的影响,这可能因手术类型而异,而且需要对患者进行纵向取样,难度较大。本项研究的目标是表征和对比IBD不同手术(包括回结肠切除术和结肠切除术)后微生物组和代谢组的变化。

 

      加州大学圣地亚哥分校的IBD生物库提供了129名受试者的332份粪便样本(50例溃疡性结肠炎; 79例克罗恩氏病)。其中21例患有克罗恩氏病患者有回结肠切除术。研究人员使用shot弹枪宏基因组学和非靶向液相色谱法进行了粪便样本中生物多样性和代谢组学的研究,然后采用串联质谱代谢组学来表征这些患者的微生物组和代谢组。

 

      研究结果显示在IBD各组患者之间的物种多样性和代谢产物多样性均存在显着差异(物种多样性:Mann-Whitney U检验;P=0.0001;代谢组学,P= 0.0043)。特别是在接受手术的受试者中,大肠杆菌的相对丰度急剧增加。与代谢物分布图相比,物种分布图能够更好地根据手术状态对受试者进行分类(平均精度为0.80 VS 0.68)。

      本项研究结果发现肠外科手术似乎减少了IBD患者的肠道微生物组和代谢组的多样性,这些变化可能会持续存在。随着时间的流逝,外科手术还会进一步破坏微生物组(而不是代谢组)的稳定性。
