European Radiology:前列腺MRI在经直肠显微超声活检指导中的应用

现在TRUS的最新进展使得使用高频超声(29MHz)成为可能,其分辨率可达70μm,比传统TRUS探头的分辨率提高了300%。

统计大约30%的前列腺癌起源于过渡区(TZ)。由于现在推荐在活检前进行多参数或双参数前列腺MRI检查,并且常规使用超声-MRI图像融合技术进行准确活检指导。在这种情况下,当与TRUS-MRI融合系统一起使用时,MRI后常规TRUS的性能在观察周围带(PZ)病变方面有很大的改善。事实上,市面上基于软件的MRI-TRUS融合系统可使90%的PI-RADS 4和5 PZ病变显影,可在简单的视觉控制下对病变同时进行实时采样。

现在TRUS的最新进展使得使用高频超声(29MHz)成为可能,其分辨率可达70μm,比传统TRUS探头的分辨率提高了300%。当作为一线影像检查时,microUS可以在大约80%的患者中显示可疑的PZ病变。作为二线影像检查,一旦在MRI上知道病变的位置,microUS可以显示MRI上可见的高达89-100%的PZ病变,而不需要TRUS-MRI融合的支持。

据我们所知临床上目前仍不清楚这种基于高频超声的方式如何对前列腺的深部区域进行成像。

近日,发表在European Radiology杂志的一项研究评估了经直肠microUS的诊断性能以及TZ-sPCa的检测率,为临床对前列腺病变的准确识别及定位提供了技术支持。

本研究纳入84名连续的男性(66±6.3岁),平均PSA水平为10.2±7.4纳克/毫升,每位患者至少有一个TZ-PI-RADS>2的病灶。所有患者都进行了MRI指导下的MicroUS和活检。评估了MRI后microUS显示PI-RADS>2的TZ病变的敏感性和特异性、TZ-sPCa的癌症检出率以及根据microUS上的可见度评估肿瘤特征。使用Cohen's kappa测试评估了读片员之间检测microUS+病变的一致性。 

在92个PI-RADS>2的病变中,71个(71/92;77%)在microUS上是可见的,进行活检时无需进行图像融合,对于21个不可见的病变(21/92;22.8%)需要进行图像融合。TZ-sPCa检测率为51.1%(47/92)。在每个病灶的基础上,MRI引导的microUS的敏感性和特异性分别为83%(39/47;95%CI:69.2-92.4%)和28.9%(13/45;95%CI:16.4-44.3%),在每个病人的基础上,MRI引导的microUS的敏感性和特异性分别为86.4%(38/45;95%CI:72.6-94.8%)和27.5%(11/40;95%CI:14.6-43.9%)。与不可见的肿瘤相比,MicroUS上可见的肿瘤表现出更大的体积和更低的平均ADC值(15.8 ± 5.1 vs. 12.5 ± 3.6 mm,0.82 ± 1.1 × 103 vs. 0.9 ± 1.4 × 10-3 mm2/s)(P = 0.02)。不可见的肿瘤显示出异质性的非特异性回声纹理。读者之间的一致性几乎完美(卡帕=0.88;95%CI:0.79-0.95)。

本研究表明,一旦通过MRI定位了TZ病灶,MRI后的经直肠显微成像可以对大多数TZ-sPCa进行识别及显示,并可实现实时视觉引导活检。
