Lancet子刊:睡眠时间不足对美国青少年早期神经认知发育的影响

考虑到睡眠不足对神经发育的不利影响,建议加强公共卫生和政策努力来促进青少年睡眠健康。

虽然美国睡眠医学学会建议6-12岁的孩子每天至少睡9个小时,但最近几代儿童的睡眠时间往往少于这个数字。由于青春期早期是神经认知发育的关键时期,本研究的目的是探讨2年多时间的睡眠不足对儿童心理健康、认知、大脑功能和大脑结构的影响。

在这项倾向得分匹配的纵向观察性队列研究中,研究者从正在进行的青少年大脑认知发展(ABCD)研究中21个美国研究地点获得了9-10岁人群样本的数据。根据每天9小时睡眠的截止时间,参与者被分为睡眠充足或睡眠不足。使用倾向得分匹配,在11个关键协变量上匹配了这两组参与者,包括性别、社会经济状况和青春期状况。如果参与者未通过基线静息状态功能MRI质量检查,或缺少倾向评分匹配相关协变量的数据,则将其排除在我们的分析之外。

从ABCD研究中检索到的结果指标是行为问题、心理健康、认知、结构和静息状态功能性脑指标,在基线和2年随访时进行评估。检查了所有合格参与者在这两年中对这些结果的分组差异。然后,对睡眠不足引起的行为变化的神经相关性进行了中介分析。

2016年9月1日至2018年10月15日期间,为ABCD研究收集了11878人的基线数据,其中8323人符合条件并纳入本研究。随访数据收集时间为2018年7月30日至2020年1月15日。在基线时确定了3021对匹配的充足睡眠-不足睡眠对,在2年随访时确定了749对匹配的睡眠对,并在两个时间点观察到两组在行为和神经测量方面的相似差异。

在这两个时间点,行为测量的组间差异的效应量是显著相关的,在静息状态的功能连接和结构措施中也观察到了类似的模式。研究发现,皮质-基底神经节的功能连接调节了睡眠不足对抑郁、思考问题和结晶智力的影响,而前颞叶的结构特性调节了睡眠不足对结晶智力的影响。

此前,一项涉及24个欧洲和北美国家的青少年睡眠模式和社会人口统计学研究发现,尽管青少年的睡眠模式在不同国家和社会人口亚组中有所不同,但睡眠不足和睡眠模式不良在大多数国家和亚组的青少年中普遍存在。这些结果建立在以往针对特定国家的研究的证据基础上,指出国家环境和社会人口群体是促进青少年健康睡眠的重要影响因素,特别考虑到睡眠不足对神经发育的不利影响,建议加强公共卫生和政策努力来促进青少年睡眠健康。
