Hepatology：HBV pgRNA用于乙型肝炎相关肝癌的肿瘤分型和预后预测

肝癌组织中复制的乙型肝炎病毒是侵入性较弱、具有转录特性的一种病毒亚型。

乙型肝炎病毒是导致肝细胞癌发病的主要原因。然而，对于乙型肝炎病毒在肝癌组织中的复制却了解很少。本研究纳入99例HBsAg阳性、病毒学得到抑制，接受肿瘤切除或肝移植的肝细胞癌患者，分析肿瘤和配对的非肿瘤肝癌组织中的病毒和细胞参数，对总HBV DNA、RNA、共价闭合环状DNA（cccDNA）、前基因组RNA（PgRNA）等HBV活跃复制标志物进行检测。


结果显示，大多数患者的肿瘤组织和非肿瘤组织中均可检测出乙肝病毒总DNA、RNA；然而，非肿瘤肝组织中cccDNA和PgRNA检出率分别为66%和90%，相比之下，部分肿瘤肝组织中cccDNA和PgRNA的检出率分别为39%和67%。对11例患者游离基因形式的X基因进行测序，结果表明11位患者中肿瘤组织和非肿瘤组织中HBV基因型不同，为HBV在肿瘤组织中复制提供了进一步的证据。肿瘤组织中，pgRNA和cccDNA的检出与无肿瘤微血管侵犯和良好预后相关。使用Agilent微阵列对基因表达进行分析，pgRNA阳性的肝癌患者，细胞周期、DNA修复水平较低，表达HBV受体-NTCP，表明肿瘤分化良好。

肝癌组织中复制的乙型肝炎病毒是侵入性较弱、具有转录特性的一种病毒亚型。前基因组RNA来源于非整合的、完整的乙型肝炎病毒基因组，是病毒复制和预后的敏感指标。
