尽管有很多研究针对心脏肌钙蛋白(cTn)I的自身抗体对实验分析的分析作用进行了描述,但尚无研究系统地检查其对临床使用中cTn分析的影响。我们确定了内源性抗体对5种不同的cTnI检测和cTnT检测的影响。

采用Siemens hs-cTnI Centaur、Siemens hs-cTnI Vista、Abbott hs-cTnI Architect、Beckman hs-cTnI Access、Beckman cTnI Access、Roche hs-cTnT Elecsys6种方法测量cTn。通过与蛋白A孵育消除免疫球蛋白后,对5种测定法(西门子hs-cTnI Vista除外)进行了重复测定。免疫球蛋白耗竭后,cTnI的低回收率(<40%)为大分子cTnI阳性。蛋白A的检测结果经凝胶过滤层析和聚乙二醇沉淀验证。

在使用西门子hs-cTnI试验的社区实验室选择的223个样本中,76%的样本显示cTnI增加(中位数为88 ng/L;四分位间距,62-204 ng/L)。在223个标本中123个(55%)观察到大分子ctni。cTnI检测方法的比较显示在切除巨cTnI患者后明显改善。

在有大分子c TnI患者中,89(72%)对cTnT的恢复没有影响,而34(28%)降低了cTnT的恢复。结果高于制造商第99百分位数的比例随c Tn分析和大分子c TnI状态的变化而变化。hs-cTnI检测之间的pass - bablok回归分析表明,无论是否有大分子cTnI,患者的斜率都是不同的。

研究认为,hs-cTnI测定的差异可能部分归因于macro-cTnI的存在。
