ACC2011:在高血压中美托洛尔琥珀酸而不是奈比洛尔升高非对称性精氨酸的水平

    背景：非对称精氨酸 (ADMA)是一种内皮一氧化氮（NO）合酶的抑制剂。我们设法确定与NO升高有关的美托洛尔琥珀酸（MET，一种β1受体拮抗剂）和奈比洛尔（NEB，一种β1受体拮抗剂和β3受体激动剂）对高血压患者的血浆ADMA的效应是否不同。
    方法:42例患者参与了为期8周的开放标签的随机化试验，给予50 mg美托洛尔或5 mg的奈必洛尔为期4周，紧随其后每日给予100 mg美托洛尔或10 mg奈必洛尔为期4周。美托洛尔组(n=19)平均年龄为57.5+12.2岁，奈必洛尔组(n=23)为59.9+8.6岁。采用ELISA法测定 ADMA和胰岛素样生长因子1（IGF-1），采用荧光素标记的KDR和CD133受体的单克隆抗体检测内皮祖细胞(EPC)，使用Omron装置测量动脉反射波增强指数(AI)。
    结果: MET组基线收缩压或舒张压为155.1±18.7/85.3±12.5 mm Hg，NEB为157.6±20.7/87.1±14.0 mm Hg；MET组基线ADMA为0.4035±0.1378，NEB组基线ADMA为0.3248±0.1228 ?mol/L。MET组ADMA在4周和8周分别增加42%和72% (P< 0.05)，NEB组无增加。第8周MET组AI增加。IGF-1和EPC在MET组和NEB组均不变。
    结论:剂量依赖性的MET升高血浆ADMA水平和AI，并未发现NEB有类似效应。
