有观察性研究发现铁蛋白和2型糖尿病有关，机体较高的铁储备可能参与了2型糖尿病的发展。然而铁蛋白不会被铁储备所影响，因此相对于其他标志物，铁蛋白并不能完全反映机体的铁代谢情况。研究人员通过观察铁蛋白、转铁蛋白饱和度（TSAT）、血清铁和转铁蛋白与2型糖尿病的发生，研究铁在2型糖尿病发病机制中的作用。此前也有类似的研究，如Diabetologia：铁缺乏导致HbA1c水平升高假象

欧洲癌症与营养前瞻性调查(EPIC- InterAct)研究中对已明确诊断为2型糖尿病并长期随访的12403例患者、以及有代表性的16154例亚群（在基线水平时被临床诊断为糖尿病）进行研究。研究人员观察了11052例2型糖尿病患者以及15182例亚群中2型糖尿病的发生与铁蛋白、转铁蛋白饱和度（TSAT）、血清铁和转铁蛋白的联系。

研究结果显示，校正了年龄、BMI、体力活动、吸烟状况、受教育程度、超敏c反应蛋白（hs-CRP）、丙氨酸氨基转移酶和γ-谷氨酰胺转移酶等因素后，较高水平的铁蛋白和转铁蛋白与2型糖尿病的风险增加有关（铁蛋白每增加100μg/L的危害比[95%置信区间]在男女中分别为1.07[1.01-1.12]和1.12[1.05-1.19]；转铁蛋白每增加0.5g/L的危害比[95%置信区间]在男女中分别为1.11[1.00-1.14]和1.22[1.12-1.33]）。在女性中，较高的TSAT (≥45% vs <45%) 与2型糖尿病风险降低有关（0.68[0.54-0.86]），但在男性中无统计学差异(0.90 [0.75–1.08])。血清铁与2型糖尿病无明显相关性。在较瘦的人群中，铁蛋白与2型糖尿病有更强的相关性。

综上所述，在研究者所构建的TSAT和转铁蛋白与2型糖尿病的模型提示了铁储备与2型糖尿病之间的潜在联系较为复杂，并不仅仅如前文中所提到的铁蛋白和2型糖尿病有关。
