含Valosin的蛋白(VCP)是一种AAA+(与多种细胞活性相关的ATP酶)普遍存在的蛋白,参与泛素-蛋白酶体系统的蛋白质降解和细胞同源性调节。遗传性包涵体肌病和额颞痴呆症(IBMPFD)是一种由VCP基因突变引起的常染色体显性遗传性疾病,在英国的患病率估计为0.66/10万,尽管全球准确的患病率仍未知。涉及骨盆和肩带肌肉的肌病是第四至第五个十年期间出现的IBMPFD8-13最常见的临床特征。 IBMPFD是一种致残状态,导致呼吸并发症或终末期痴呆死亡。其他报告的表现包括面肩肱肌无力、远端肌病、肌萎缩侧索硬化症(ALS)、帕金森病、遗传性痉挛性截瘫、Charcot-Marie-Toth病、亨廷顿病和心肌病。这种广泛的表型需要一个多学科团队对患者进行护理,如果存在非典型症状或不明确的家族史,则其诊断具有挑战性。

IBMPFD命名法存在缺陷,由于结构变化,VCP突变损害自噬和内吞转运,并增加ATP酶活性。作为一种罕见情况,MSP自然史信息有限,详细的基因型-表型关联尚未建立。本研究旨在(1)描述一个大型国际队列的VCP基因突变患者的临床和遗传学特征,(2)研究基因型-表型关联。本文发表在《神经病学,神经外科学和精神病学杂志》上()。

这是一项国际描述性回顾性研究,收集了MSP患者常规临床护理访问的数据。来自24个国家的52个中心参加了研究。纳入标准为:(1)VCP基因中致病性(P)/可能致病性(LP)变体的杂合子患者(转录参考NM_007216.3)(2) 临床案例提供了足够的数据,以回答以下一些临床问题:发病年龄、遗传诊断、发病时的体征/症状、临床进展、门诊状态和辅助测试结果(如果执行)。如果基于纳入标准第(2)项的电子分析和临床判断支持疾病因果关系和/或家系表明显性疾病,则包含具有未知意义变异(VUS)的患者。收集所有患者的临床和家族史数据后,计算美国医学遗传学和基因组学学院和分子病理学协会(ACMG)对每个变异的标准。

从疾病发作开始的体征/症状发展时间

研究包括255名患者(70.0%男性)。平均年龄为56.8±9.6岁,平均发病年龄为45.6±9.3岁。平均诊断延迟为7.7±6年。50%的患者在发病时出现对称性下肢无力,并进展为全身性肌无力。其他常见症状为通气功能不全40.3%、PDB 28.2%、自主神经障碍21.4%和FTD 14.3%。共鉴定出57种基因变异,其中18种以前未报道过。c、 464G>A(p.Arg155His)是最常见的变体,在28%的患者中鉴定。全职轮椅使用者占19.1%,从发病到轮椅使用者的中位时间为8.5年。变体c.463C>T(p.Arg155Cys)显示发病较早(37.8±7.6岁),且出现轴和上肢无力、肩胛翼和认知障碍的频率较高。强迫肺活量(FVC)低于50%是全职轮椅使用者的风险因素,而FVC<70%和全职轮椅使用者与死亡相关。

两种最常见变异之间的体征/症状频率

2004年,在出现肌病、PDB和FTD三联症的患者中发现了六种错义致病性变体,发现VCP基因是IBMPFD的病因。随后的队列证实了这种三联症在VCP基因突变的患者中很常见,但也将表型扩展到涉及中枢神经系统、运动神经元、感觉和/或运动周围神经和骨骼肌。本文的研究证实,肌肉无力(影响下肢和/或上肢的近端或远端肌肉)是起病时的主要症状,使MSP成为具有挑战性的诊断,而患者可分为肢体束带肌无力、远端肌病或肩胛腓骨综合征。8%的患者在发病时表现出不对称肌无力,这将诊断挑战扩展到运动神经元疾病和运动神经病变。无论起病时的肌无力模式如何,大多数患者在最后评估时表现出全身性肌无力和萎缩。值得注意的是,在疾病的前5年内出现了轴性肌无力、肩胛翼和运动神经元体征,使早期的鉴别诊断过程复杂化。

在平均5.2年的随访后,12名致病性变异携带者没有出现症状。VCP的外显率变异性尚未完全理解。在本研究中,大多数无症状携带者尚未超过其家庭预期的最高发病年龄,可能携带VCP基因变体但未进行基因测试或未由参与本研究的医生跟踪的家庭成员未被纳入分析,从而限制了对该疾病外显率的解释。包括大量无症状携带者并对其进行随访的研究可以调查外显率是否降低是VCP的常见特征,以及亲属的发病年龄是否可以用作无症状携携带者预期发病年龄的估计值。需要进行更多的研究,以调查不完全外显率是否根据临床表型而变化,并检查与不完整外显率相关的其他因素,如遗传修饰因子或环境因素。
