BJR:核磁共振成像可用来检测癌细胞的骨转移

    据英国放射学杂志8月刊文章中结果显示:DWI是前列腺癌或多发性骨髓瘤病人全身核磁共振成像的首选技术。“同传统的全身STIR 序列相比,DWI可能提高恶性肿瘤中骨疾病的检测效果,这可能和全身肿瘤的影像学报告的内容有关。”陶顿市慕斯格雷夫公园医院放射科Paul Burn和其他作者写到。

    越来越多的证据显示运用全身核磁共振成像检测恶性肿瘤病人的骨转移瘤,这些恶性肿瘤(包括前列腺癌、乳房癌和多发性骨髓瘤)有转移到骨骼的倾向。一项研究表明全身核磁共振成像对骨转移瘤的检测灵敏度和专一性≥90%(J Mag Reson Imaging 2011; 34: 128-135)由于核磁共振成像扫描已变得更加先进,成像报告的可利用性更加提高了。

    按照常理,核磁共振成像仪运用了T1加权或STIR序列。目前,DWI受到越来越多的成像专家的喜爱。然而,DWI作为STIR的改良成像技术已经在比较研究中得到证明。作者指出:“很显然,高病变-本底对比度能提高病变的显着性和检测能力。”为了比较显着性的病变的和两个影像学报告,Burn和他的同事们回顾44个骨转移瘤病人的全身磁共振影像,这44个病人包括22位是胰腺癌病人、7个多发性骨髓瘤病人和13个乳腺癌病人。

    对于每一次扫描,研究者选择了多达4个大面积骨病变,通过T1-加权鉴定并确定了每个病变的范围。一旦有可能,研究者便能在身体的不同影像区域确定病变靶位。对于弥散加权成像序列,作者解释到:他们选择b值为800作为获得病灶信号(T穿透效果)和更大的弥散加权的平衡。发病区域和本底信号强度是通过DWI(b=800s/mm2)和STIR来衡量,病灶信号/背景比率通过每位病人和每一种恶性肿瘤来评价。

    对前列腺病人,DWI产生的病变/本底比率为3.91,而STIR产生的为2.31(P-0.0001),在24位病人中有22位,DWI形成的比率较高。对于多发性骨髓瘤患者,DWI产生的病变/本底比率7.59,STIR产生的病变/本底比率3.7。在7位患者中有6位,DWI形成的比率较高。13个胸腺癌病人DWI产生的病变/本底比率4.13,STIR产生的病变/本底比率4.26没有显着性差异。但是,有九名病人DWI形成的比率较高。“结果显示:对于胰腺癌病人和多发性骨髓瘤病人的骨疾病检测,全身DWI技术相较于传统STIR成像技术有更高的灵敏度。”作者写到。

    “很显然,STIR和DWI所选择的参数,特别是b-值,都有可能影响病灶的显着性。”作者补充到。“尽管对于大多数病人STIR显示出较低的病灶显着性,但对于少数情况,它又显示出较高的病灶显着性。STIR还有其它的优势,包括它能给出结构信息还能降低人工杂质的灵敏性。”研究的局限性包括:样品量小和对受试者组间和组内差异之间测量的缺乏,另外,患者群体的差异性也有一定的局限性,所以作者呼吁一项基于均一病人群体的研究是非常必要的。
