三分之一乳腺癌患者最终都发展远处器官的转移,伴随死亡风险的增加。 因为早期肿瘤比较小,对药物治疗最敏感,所以早期肿瘤的检测是制定适当和有效的治疗干预的关键。虽然现在有多种成像技术,包括MRI,使用于乳腺癌检测和临床管理,它们既不能够检测特定的癌症类型也不能发现早期肿瘤的生长。

癌症扩散的最早迹象被称为微转移。正如其名,它们往往太小以至无法用标准的筛选方法检测。Lu博士的研究小组利用一种特殊的化学对比剂结合MRI方法检测到微信号分子变化。 在这个化学对比的方案中,研究团队开发出一小段只含有五个氨基酸的肽链,并贴上了微小磁铁。选择这个肽链是因为它的倾角使其能结合癌细胞周围的蛋白质基质的结构——纤维蛋白-纤连蛋白复合物。更重要的是,该复合物的纤连蛋白部分是细胞过渡到癌细胞时特意表达的,并在细胞生长,迁移和分化中有重要作用。纤连蛋白通常与高风险,预后差的乳腺癌有关。在采集描绘微转移图像时,对比剂内金属分子在MRI过程被磁化并增强溶液与目标蛋白分子结合的图像。




研究人员在引入乳腺癌细胞的小鼠身上测试了这种方法。经过两个星期的等待期,研究人员注入造影剂,进行MRI扫描,检查转移肿瘤,包括微小转移肺、肝、淋巴结、肾上腺、骨和小鼠的大脑。图像的分析表明,所使用的对比剂结合了几乎所有的纤维蛋白—纤连蛋白复合物,从产生强信号和延长的的微转移和肿瘤增强图像。结合显微成像(cryo-imaging)的方法和MRI,研究人员验证了MRI技术可以检测到甚至小于0。5mm的骨微转移。

在使用此对比剂深入研究之前,研究人员已经检测了对比剂在人体内代谢消除的时间。他们的测试表明,对比剂容易从体内清除,在组织中保留较低水平,表示如果将其用于临床开发,是具有安全性的。

总得来说,这个方法能针对性地进行临床高危癌症的早期成像,从乳腺癌的复发率和转移癌的恶性后果考虑,这个研究发现意义尤其明显。
