未来人类宝宝或可通过3D打印的子宫出生

发着绿光的小老鼠蹲在鼠妈妈身边,看上去就像货架上的儿童玩具。但它会抽抽鼻子,甩甩尾巴,用黑溜溜的圆眼睛看着你。尽管有着半透明的荧光躯体,它仍然是一只货真价实的老鼠宝宝。

比起特殊的皮毛,它的来历更加不同寻常——这只色彩炫目的荧光小鼠,是3D打印的卵巢排卵孕育的。



瘢痕化的子宫内膜修复过程

“这表明生物的卵巢具有长期的、持久的功能。”实验负责人特蕾莎·伍德拉夫说。她是位生殖科学家,也是美国西北大学范伯格医学院的女性健康研究所主任。“使用生物工程替代器官移植,创造器官结构功能,并恢复组织的健康,这是再生医学生物工程的圣杯。”

“再生医学”这门新学科,从名字到研究目标,都充满了浓浓的科幻感。研究者用3D打印技术制造过气管、脊髓、子宫内膜,现在正在试图打印心脏或肝脏。

“在再生医学领域,中国的一些项目目前处于世界前列。”中国科学院遗传与发育生物学研究所再生医学研究中心(以下简称再生医学研究中心)主任戴建武对中国青年报·中青在线记者说。

戴建武的团队从事再生医学研究10余年。从2013年起,他们与南京鼓楼医院合作,用干细胞复合功能支架材料,修复瘢痕化的子宫内膜。最新的数据是,已经有13名女性,生下了14名健康的宝宝,另有6个胎儿正在孕育中。

也许有一天,人类宝宝会通过3D打印的子宫出生

在美国西北大学的实验室里,7只雌鼠被摘除了卵巢。伍德拉夫团队给这些卵巢消毒,随后保存了卵巢组织,并分离出支持不成熟卵子的激素生成细胞。

3D打印机开始工作,一种水凝胶作为基本材料,用于打印出小鼠卵巢的基本结构。随后,伍德拉夫和她团队中的研究者,将之前分离出来的雌鼠卵泡,填充进刚打印出来的卵巢里。卵泡已通过基因编辑注入绿色荧光蛋白,新生的小鼠将拥有炫目的荧光,和其他小鼠区分开来。

这些由生物材料制成的卵巢,被外科手术移植回小鼠体内。最终,受试母鼠生出了幼崽,甚至顺利分泌出了乳汁。

这项实验由西北大学麦考密克工程与实用科学学院和西北大学范伯格医学院联合开展,团队的成员全是女性。实验结果发表在2017年5月16日的《自然-通讯》上。

在一个全女性团队中参与研究女性健康问题,让助理教授瑞米尔·沙阿很受激励。她感慨团队成员表现出的幽默感:“特蕾莎和我开玩笑说,我们是这些幼崽的祖父母。”实验的结果也让很多人觉得,也许有一天,人类的宝宝也会通过3D打印的卵巢和子宫出生。

在世界范围内,恢复女性的生育能力目前已成为再生医学的主要目标之一。

5月21日,南京鼓楼医院与中科院遗传发育所联合组织了一个“爱与希望”亲子联谊会,14位“再生医学宝宝”中的8位参加了活动。

14个宝宝当中最大的已经快3岁了,最小的还未满月。在成为戴建武团队再生医学研究的临床受试者之前,这些孩子的母亲,都因为重度子宫腔粘连、子宫内膜瘢痕化等疾病,常年奔波在求医的路上。

据再生医学研究中心介绍,目前,我国女性不孕症患者约2500万人,子宫腔粘连等子宫内膜损伤患者占很大比例。子宫内膜瘢痕化、供血不足,最终会导致受精卵无法着床发育。重度患者在临床上会被判定为终身不育。

“我们的这项技术,是利用胶原蛋白作为支架材料,复合患者自体骨髓干细胞,或中科院北京干细胞库的脐带间充质干细胞,进行瘢痕化子宫内膜的修复。”戴建武介绍。

在内窥镜的镜头之下,干细胞和生物材料开始在子宫里原本斑驳不平的创面上生长和修复,瘢痕的部分逐渐被填满、抚平。

几个月过去,瘢痕消失,内膜上长出了新的腺体,受精卵开始着床。2014年7月,国内第一例“再生医学宝宝”出生了。

血液开始流通,卵细胞发育成熟并排出,荷尔蒙开始循环

戴建武很早就开始关注美国西北大学的荧光小鼠实验。

“在技术上,我们目前也是可以做到的。之所以没有做,是考虑到这个实验在临床上意义其实不大。”戴建武解释,尽管小鼠的卵巢是以3D技术打印的,但是卵巢里面塞着的卵泡,以及各类能产生卵细胞的激素成分,实际上都是从小鼠原有卵巢中提取出来的,原本就生殖功能完好,并不存在病理性问题。

在他看来,这个实验“在目前的临床应用中价值不大”。他在临床上遇到的不孕不育患者,卵巢往往都不具备完整的功能。用3D打印雌鼠卵巢,并没有对再生医学的瓶颈和难点实现真正的技术突破。

“再生医学现在真正的难点,是微环境的调控问题。意思是,我们怎样让干细胞在人体之内,按照我们设定好的程序,不受干扰地长成我们想要的组织或器官。这个问题,目前全世界都没有解决。”戴建武说。

目前,干细胞可以在实验室中,被分化成神经细胞、皮肤细胞等各种临床所需要的功能细胞,但如果在人体内培植,就很容易受到人体产生的各种诱导分子的干扰,一不小心就“长歪了”。

在体外培植的器官,就算是使用同样来源的干细胞来培育,移植回体内的时候,仍然需要适应。戴建武举了个例子:“就像自家的孩子打小在外面养着,大了才抱回来,和家人肯定会有隔膜。”

伍德拉夫和她的团队也承认,打印小鼠的器官,距制造人类器官还有一段距离。人类的卵巢远远大于老鼠的卵巢,结构也更加复杂。目前使用的材料,如果要支撑打印人体器官,强度仍然不够。

在瑞米尔·沙阿看来,荧光小鼠实验中,真正与众不同的部分,是3D打印中所使用的“墨水”,也就是支架和材料的结构。她们使用的是生物性水凝胶,这是一种安全的生物材料,由被破坏的胶原蛋白制成。

“大多数水凝胶都很脆弱,因为它们大部分是水,而且往往会自行坍塌。”沙阿说。伍德拉夫团队找到了一个恰当的实验温度,在这个温度下,实验中所用的水凝胶能够自我支撑而不会崩塌,这使得多层结构的形成成为可能。“这是第一个证明支架结构对卵泡生存有影响的研究。”

在美国西北大学的实验室里,研究者开始用这些水凝胶“墨水”,给小鼠的新卵巢搭建“脚手架”。它被编织成一层层交错的结构,中间的空隙用来留给细胞通行,以便将来跟小鼠的体细胞相结合。她们小心翼翼地控制着这些水凝胶细丝之间的距离,控制层间的前进角度,打印出不同的孔隙大小和不同的孔隙几何形状。

当3D打印出的卵巢被小鼠的体内循环系统接纳后,这些孔隙成为交流的通道,血液开始流通,卵细胞发育、成熟、排出,荷尔蒙开始循环。

最终,7个人工卵巢里,有3个让受试小鼠正常怀孕,生下了那些熠熠生辉的幼鼠。

未来,人类的器官就像壁虎的尾巴一样

对伍德拉夫来说,实现生育功能,并不是她的团队对卵巢进行研究的最终目标。给老鼠移植3D打印的卵巢,也仅仅是她们团队研究的一个开始。

“我们的想法是,一个年轻的癌症病人接受化疗前,将她的卵巢组织提取出来,分离卵泡。当她的癌症被治好了,我们可以给她打印一个新的卵巢。” 伍德拉夫在接受媒体采访时说,“妇女的医疗保健需求,远远超出了生殖系统的范围,涉及所有医学领域。现在是时候将这一基础科学知识,纳入到全国最全面的妇女医疗计划中去了。”

她发现,很多人认为卵巢仅仅是具有生育能力的,但卵巢其实是“女性健康整体体系的一部分”。2006年,伍德拉夫创建了女子健康研究所,致力于推进基于性别的生物学领域发展。

对干细胞和组织工程等领域的研究,正推动着再生医学的发展。对研究者来说,未来的目标,是希望人类的器官就像壁虎的尾巴一样,砍掉了,原处会长出一个新的来。

“再生医学的前景很可观。未来有一天,或许需要什么器官,就能用干细胞制造出来。” 解放军总医院(301医院)的陈海旭博士对中国青年报·中青在线记者说。在陈海旭所在的研究团队,利用自体骨髓干细胞治疗I型糖尿病的研究,已经到了临床试验阶段。

这项技术还能用于治疗Ⅱ型糖尿病患者后期并发症,由于组织修复能力减退,伤口时常不易愈合。基于再生医学的疗法能够促使创面愈合,干细胞被用于促进新生血管的生成及血流供给,改善下肢缺血的症状。

陈海旭博士还提到了一种利用自体骨髓干细胞治疗溃疡性结肠炎的方法。他们在肠镜下,将干细胞移植到结肠黏膜的溃疡周围。90多天后,他们观察到,和患者纠缠了3年多的顽固性溃疡,已经基本愈合了。

中科院再生医学研究中心最新的成果是,利用智能生物材料构建微环境,对受到损伤的脊髓神经进行修复。戴建武研究团队设计和制备了适合脊髓损伤修复的有序神经再生胶原支架,引导神经细胞有序生长。这项研究已经于2015年1月进入临床实验阶段。

再生医学涉及的3个概念,包括干细胞、生物材料和细胞中的生长因子。2008年6月,一位西班牙患者接受了世界首例自体干细胞培育的人工气管移植手术,恢复状况良好。当时采取的方式,是将患者的自体干细胞,与其他捐献者的气管组织在模拟环境的生物反应器中一起培养,最终移植。

3D打印技术的发展,给了再生医学研究者新的体外器官构建思路。理论上,基于3D生物打印出的器官,可以完美定位细胞,最终打印出功能完备的组织器官。

伍德拉夫实验室也有很多“疯狂的计划”,其中之一,是寻找一种“能创造持久器官的材料”,既可以“快速固化成型”,又能“符合组织器官相应的力学强度”,还可以与人体环境良好相容。接下来,她们准备打印“迷你猪”的卵巢。

“这在未来,不会太遥远。”伍德拉夫仍然保持乐观,“也许距离人们不到10年的时间,只是我们还没有到那里。”
