癌症伴脊柱压缩性骨折:球囊后凸成形术治疗有效

  《柳叶刀·肿瘤学》(Lancet Oncol)最新发表的一项研究表明,球囊椎体后凸成形术治疗癌症伴脊柱压缩性骨折是安全有效的,可迅速减轻疼痛,并改善脊柱功能。

  该研究是一项随机对照临床试验,在欧洲、美国、加拿大和澳大利亚的22个医学中心纳入21岁以上、1~3个椎体出现疼痛性压缩性骨折的癌症患者。患者被随机分配到后凸成形术组或非手术组(对照组)。主要终点为1个月时,用Roland-Morris功能障碍问卷(RDQ)评分评出的患者背部特异性功能状态。对照组患者在1个月后被允许交叉到手术组接受后凸成形术。

  结果显示,在2005年5月至2008年3月,有134例患者被纳入研究,70例进入手术组,64例进入非手术组,1个月时两组分别有65例和52例患者的资料可评估。RDQ平均得分改变:手术组从基线的17.6分到1个月时9.1分(平均减少8.3分,P<0.0001),对照组从18.2分到18.0分(平均减少0.1分,P=0.83)。1个月时,手术治疗的效果为RDQ减少8.4分(P<0.0001)。在第一个月内最常见的不良事件是背痛(手术组:4例/70例;对照组:5例/64例)和有症状的椎骨骨折(2例 对 3例)。
