高危非急性颈内动脉闭塞血管内治疗的研究进展

非急性颈内动脉闭塞（non-acute internal carotid artery occlusion，NA-ICAO）目前主要的治疗手段包括药物保守治疗、搭桥手术、颈动脉内膜切除术（carotid endarterectomy，CEA）和血管内治疗（endovascular treatment，EVT）。

血管内治疗的发展
2005年，Terada等首次应用近端血流逆转保护技术，成功实现第一例NA-ICAO患者的血管再通，且术中、术后并未出现远端栓塞、出血等并发症。此后便陆续有NA-ICAO成功开通的报道，提示EVT可能是高危NA-ICAO可以尝试的一种治疗方式。
随着手术室设备和操作技术的进步，目前CEA联合EVT的复合手术正逐渐兴起。这类复合手术首先利用CEA开通钙化或纤维化严重的NA-ICAO近端，然后用EVT开通远端的闭塞，两者联合应用可更有效地提高颈动脉长节段闭塞的再通率，并且降低围手术期并发症的发生率。不过，该方法对技术和手术室的条件要求较高，目前推广较为困难。
此外，国内莫大鹏团队提出EVT治疗NA-ICAO的新方法——阶段性血管成形及支架置入术，即先对NA-ICAO患者行血管初始再通和小球囊扩张术，4～6周后再行血管成形及支架置入术，结果提示该方法可减少高灌注等手术并发症的发生和所需的颈动脉支架数量，期待未来有更多的研究加以验证。

血管内治疗术前评估维度及方法

关于EVT治疗NA-ICAO的病例精准选择方面尚无定论，目前多是针对症状性、药物治疗后仍有卒中发作的高危NA-ICAO患者进行EVT治疗。

血管内治疗的安全性
既往研究表明，EVT治疗NA-ICAO的并发症发生率在各个研究中心存在较大差异。EVT相关的并发症包括栓塞、脑出血、颈动脉海绵窦瘘、动脉夹层或穿孔、假性动脉瘤、心动过缓等。由于各个中心对并发症的定义、判定并发症严重程度的标准不同以及对研究病例的选择偏倚，可能会造成各研究对术后不良事件的判定存在一定异质性，故各研究并发症发生率可能不具可比性。2019年本团队进行了EVT治疗NA-ICAO（包括复合手术）的系统分析，结果显示， EVT用于NA-ICAO的治疗可能是安全可行的。

血管内治疗的有效性
EVT治疗NA-ICAO的报道中，各研究的手术再通率在58.5%～88.8%之间。近年多个研究认为血管内再通可改善慢性颈内动脉闭塞患者的认知功能。
目前的研究仍存在一些局限性：EVT治疗NA-ICAO的研究大多为单中心、小样本的研究，故仍需大规模、更高质量的研究以提供更高级别的证据。
目前的研究提示，高危NA-ICAO患者采用EVT或EVT联合CEA复合手术，可改善患者的脑灌注，降低同侧缺血性卒中风险，同时围手术期严重并发症发生率较低，手术有效性和安全性被逐渐接受。
但目前NA-ICAO在手术患者精准筛选方面仍无统一标准，缺乏全面的认知维度、步态、神经功能以及脑灌注定量检测等系统性的评估，手术过程尚缺乏规范的技术指导。因此迫切需要开展更大规模、更高质量的多中心研究，以明确EVT治疗NA-ICAO的适应证，提高再通率，寻找可能影响EVT安全性和有效性的危险因素。
